📈 What data are you using in your PCN to improve health outcomes for your patients? The PCN DES specification states that a "PCN must seek to improve health outcomes for its population using a data driven approach and population health management techniques in line with guidance and the CORE20PLUS5 approach." There are a lot of of different datasets which can be used to help deliver this service requirement with the SHAPE Atlas Tool being one of those.
