Double-Checking Prescriptions: A Close Call Remembering my first year on the floor? Fresh out of nursing school, confidence high. Then came a medication order for a new patient – a high dose of a diuretic. Everything seemed legit... until I double-checked the patient's weight and saw it was WAY too low for that dosage. Here's the Problem – Not Everyone Double-Checks. Most new grads I see don't: Question Orders: Feeling unsure about a dose or unclear instructions? Ask! Verify with the Provider: A quick call can clarify any inconsistencies and ensure patient safety. Document Everything: Note the time, who you contacted, and any changes made to the order. Too Much Rushing, Not Enough Safety: NCLEX® emphasizes the importance of verifying medication orders. A missed inconsistency could have serious consequences. Future Nurses, Remember, when in doubt, ALWAYS clarify.
