A patient, a 65-year-old male with a history of #smoking, presented with symptoms of cough and weight loss. Imaging studies revealed a primary tumor in the right lung with multiple metastases to the bones and #liver. Biopsy confirmed non-small cell lung cancer (NSCLC). The patient's performance status was relatively good, despite the advanced disease. The medical team considered the following factors in their decision-making process: Patient's Overall Health: The patient was active and had no significant comorbidities, which made him a candidate for systemic therapy. Stage of #Cancer: Given the multiple sites of metastases, the disease was classified as stage IV. Symptoms: The patient's symptoms were manageable and did not significantly impair his daily activities. Patient's Preferences: The patient expressed a desire to pursue aggressive treatment in hopes of prolonging life. Life Expectancy: The patient's life expectancy was estimated to be limited due to the advanced stage of cancer, but potentially extendable with treatment. Treatment Side Effects: The potential side effects of chemotherapy were discussed, including the risk of fatigue, nausea, and decreased immune function. Effectiveness of Treatment: The oncology team believed there was a chance to control the disease and possibly shrink the tumors with chemotherapy. Palliative Care Benefits: Palliative care was considered to manage symptoms and maintain quality of life alongside treatment. Healthcare Resources: The patient had access to a comprehensive cancer care facility. Psychosocial Factors: The patient had strong family support and a positive outlook, which are important for coping with treatment and its side effects. After thorough discussion, the decision was made to proceed with a combination of #chemotherapy and targeted therapy, aiming to control the disease and maintain the patient's quality of life. The patient was also enrolled in a clinical trial for a new #immunotherapy agent, providing access to cutting-edge treatment.

The oncology symptom/treatment course is so challenging, especially for patients that live far from such specialized cancer centers. This impressive work furthers our understanding of how to do it effectively and what the potential outcomes might be. The authors state: "Overall, a combined acute and surveillance-based subacute hospital-at-home program in rural areas can decrease care utilization, reduce and prevent symptom escalations, and improve patient experience. Implementing programs such as Rural HH should focus on enhancing the use of technologies to proactively improve patient monitoring, decrease patient travel burden, and facilitate communication among the broader care team. Reaping the benefits of this innovative home-based care model will require permanent adoption of value-based payment models." MD Anderson Cancer Center required a $10,000 deposit at my initial #patient registration session, after which I would see an #oncologist & an oncology surgeon. They recommended a CT scan to determine the extent of my non-healing wound, a basal cell carcinoma. The wound had been seen by 6 or more #healthcare professionals in urgent care, a wound clinic, a #pcp , again in #woundcare and MD Anderson. I have #medcare. a second PCP, again in #wound care and MD Anderson. I have #medcare and here in Cyprss TX @harrishealth system financial assistance; #medicaid has denied my three applications. During Now, four months later, after sending my case to Harris Health System, largely due to my selfpay status and the uncertainties in treating the wound/ #cancer, MD Anderson is refusing my requests for refunding the $10,000 less costs associated with _minimal care_ services performed. MD A has referred me to Harris Health's LBJ Hospital for on-going treatment. MD Anderson Cancer Center's avarice has #polluted its #mission and corroded its #values. Fee-for-service is its #lifeblood; that #addiction precludes a #patient centered, #valuebasedhealthcare mindset and subsequent noble, lofty #performance. MD Anderson should dread the rise of #ai. Using AI, we patients will be able to find novel treatments for #cancers, #heartdisease and other #chronicdiseases, alternative #medications (such as fenbendazole for cancer) and options when choosing #healthsystems and #providers. #health #consumers will wield unprecedented decision-making power the likes of which has never before existed.

*Oncology team leads virtual clinic innovation at East Sussex NHS Hospital Trust* As healthcare organisations strive to implement virtual clinics and patient initiated follow up (PIFU) pathways, it’s invaluable to hear directly from clinical teams. Oncology co-ordinator Izabela Przyczyna shared her experience implementing a Self-Support Management Programme for prostate cancer patients - a first at East Sussex Healthcare NHS Trust. Using PKB (known as My Health & Care Record in Sussex), patients are empowered to manage their health post-treatment, reducing anxiety and fostering a more patient-centric approach. 68% of consultant haematologists are reporting staff shortages according to research conducted by Blood Cancer UK. Professionals from across the workforce have told them that nursing levels have not adjusted to the higher level of care needed by patients both on wards and as outpatients. Here are some key highlights: 👉🏻 The DISCO trial has looked to incorporating and personalizing physical activity into patient care which seems to be part of a growing base of evidence 👉🏻 The IMPACT study looked at how an improved approach to patient education led to an increase in adherence to oral therapies in advanced breast cancer 👉🏻 A study around the Untire app looked at how cognitive behavioral therapy, delivered digitally, can positively impact fatigue and quality of life “Personalised digital health technologies tailored to reflect the unique medical history of the patient, their individual risk factors and their literacy skills, will offer the potential for patients to be more proactive in their own care. Importantly, these tools have the capability of reaching patients worldwide and in some instances, could help alleviate the burden on overwhelmed healthcare systems.”
