Controversial but an honest take from me. I accepted very quickly when I was a GP Trainee that no one is going to help the NHS, apart from the people working in it. When I heard about the vast issues, the decade of attempts sometimes by passionate people to address issues and how it went nowhere. I realised that people burn out, lose hope and give up often before anything is achieved. I decided then that whatever issues I found, I would work on myself rather than waiting for an answer from higher up, as people had waited their entire careers and gotten nowhere. After establishing a baseline of efficiency in common primary care issues with document workload, abnormal result processing and script signing, my attention turned to upskilling staff and segmenting work streams and ensuring they are done to a high standard at each level. Once we did this we moved onto supporting with pharmacy services and most recently have begun to often software solutions, in particular our GP Triage software which aids in acute triage while helping manage patient demand. We offered our services often directly to some practices and via ARRS funding to others. We always had 1 consistent issue which was some things such as consultations, or complex reviews would have been ideally provided by GPs, and for direct clients this was fine, but it posed an issue at PCN level. We have over 100 GPs registered with us, who work on our direct engagement clients. But for the first time we will be able to offer GPs work via PCN funding given the recent guidance. Not only will this hugely benefit practices and PCNs but it will also help new GPs who have been struggling to find work. A vital factor for certification board success: Engaging stakeholders Our mission as certification bodies is to maintain standards and encourage excellence within our fields. Crucial to this mission is stakeholder engagement. Importance of engaging stakeholders: - Assures certifications remain relevant - Builds trust and credibility - Encourages continuous improvement Ways to enhance stakeholder engagement: 1. Identify key groups (e.g., nurses, employers, educators) 2. Set up regular feedback channels 3. Include stakeholders in setting standards 4. Clearly communicate changes and updates 5. I genuinely can’t believe the way primary care is going in the UK. I know alot has been said recently about the ARRS roles, but the work landscape for primary clinicians is pretty much nonexistent at the moment with so many out of work. If I were a GP trainee right now I’d be looking to change my training to a different specialty next year. As a GP I’m always keeping my options open and at times recently, have very much contemplated leaving primary care altogether for roles in health tech/digital health I’d be suited to (such as MD roles). I’ve yet to find one that really ticked the boxes that would mean I’d be able to make a decent impact and that was suited to my needs. With how unstable the job market is, stability is golden at the moment, but with the very little work that is available, I’ve seen many examples of employers/primary care organisations taking advantage of the crisis to shaft clinicians further when they are down. I’ve not taken the plunge yet but the market really worries me. Unless the ARRS initiative is reversed OR primary care is given proper funding (can’t see either happening), I don’t think this is just a normal periodic trough in the primary care jobs market this time. To all GPs, don’t rely on primary care for your work and stability anymore. As for me, it’s been anxiety provoking and the lesson for me is that building on shifting sands is never a good idea.
