How do you quantify reablement in your service? Reducing the need for further ill health supporting people using your service to be as independent as possible ? Do you score the support you offer to people using your service? When I was a service manager for a reablement service we scored : 1 = independent 2= needs prompts or supervision 3= needed some support 4= totally dependent This is how we measured progress with activities of daily living and, helped provide evidence of, no longer needing support or, the need for ongoing funded support. My personal measures are; 1= can do up my own bra 2= can put my own hair into a ponytail 3= can undo my mascara and put it on with my dominant hand 4= can type and use my mouse to begin audits of care plans: How ever you record levels of independence and monitor progress , being so dependent these last few weeks has reminded me how important it is to feel like you can do some things for yourself. People mean well to do for you but, it means the world to do small things for yourself ❤️
