What are the Symptoms of NASH? Fatty Liver can be mild and symptomless, but if it progresses to NASH, it can sometimes cause symptoms like unexplained lethargy, loss of appetite, vague abdominal pain and may even lead to liver damage. Complications of NASH: It increases the risk of cirrhosis-related liver failure and liver cancer, which could require a liver transplant. The average life expectancy for someone with cirrhosis of advanced liver disease is about 9 to 12 years. However, NAFLD (Non-Alcoholic Fatty Liver Disease) greatly increases the risk of cardiovascular disease and decreases life expectancy in adults. The combination of acute kidney injury (AKI) and liver cirrhosis presents a challenging prognosis for patients who are not eligible for liver transplant. It is often difficult to decide whether dialysis should be offered to these patients. We have noticed that mortality rates remain consistently high regardless of the cause of AKI (acute tubular necrosis vs. hepatorenal syndrome). Still, there is a lack of tools to guide clinicians when faced with the question to offer or not renal replacement therapy. One important finding is the low rate of palliative care consultation for these patients, indicating a need to initiate these discussions early in the disease course. https://lnkd.in/eeKtT6hU

The use of sodium glucose cotransporter 2 (#SGLT2) inhibitors in patients with chronic #kidney disease with #proteinuria is associated with a decreased risk of end-stage renal disease and all-cause mortality. Several small studies have also shown improvements in proteinuria in patients with #lupus #nephritis (LN) treated with SGLT2 inhibitors. This creates expectations that the treatment use of SGLT2 inhibitors in the long term will help to mitigate #cardiac and #renal complications commonly seen in LN patients. 🔗https://lnkd.in/dSFTmTMS

📢 New Publication Alert: Liver Transplantation for Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD) 📢 The global rise in obesity and metabolic syndrome has led to a significant increase in liver transplantation (LT) for MASLD. Patients typically present with advanced age, higher BMI, and metabolic comorbidities such as diabetes and hypertension, making comprehensive pre-transplant evaluation crucial. 🔑 Key Highlights: MASLD patients often face longer operative times and more post-op challenges, including higher rates of metabolic syndrome and cardiovascular events. Despite these risks, survival rates at 1, 3, and 5 years post-transplant are comparable to those of other liver disease etiologies. A multidisciplinary approach is essential. Addressing comorbidities, managing cardiovascular health, and monitoring steatosis recurrence can enhance long-term outcomes and improve patient well-being. Kidney failure is a condition in which the kidneys cannot adequately clear waste and excess fluid from the body. This may occur as a result of decreased or complete loss of kidney functions. Kidney failure can generally be of two types: acute kidney failure and chronic kidney failure. 1. Acute Renal Failure: - This condition usually occurs suddenly and develops rapidly. - The most common causes include severe blood loss, serious infections, medications that damage kidney tissue, or exposure to toxins. - Treatment usually involves addressing the underlying cause and supporting kidney function. Other measures may be taken to restore fluid balance, regulate electrolytes, and support the kidneys. 2. Chronic Kidney Failure: - Chronic renal failure is a progressive process that usually lasts for years. - Common causes may include diabetes, high blood pressure, genetic factors, inflammatory kidney diseases and polycystic kidney disease. - Treatment may often include lifestyle changes, medication, dialysis and kidney transplantation. Diagnosis and intervention in the early stages can slow the progression of the disease. A number of approaches can be used to treat kidney failure, depending on the patient's condition and level. The treatment plan should be determined by a nephrologist (kidney specialist). Methods such as diet, fluid control, medications, hypertension management and dialysis can be used in this process. Kidney transplantation is a long-term treatment option for some patients with advanced kidney failure. An important point to remember is that kidney failure is manageable with early diagnosis and treatment. Honoured to participate in this study published in The Lancet exploring the use of spleen stiffness measurement (SSM) at 100 Hz as a non-invasive diagnostic tool for clinically significant portal hypertension (CSPH) in patients with compensated advanced chronic liver disease (cACLD). These research found that SSM, when combined with liver stiffness measurement (LSM), platelet count, and BMI in the NICER model, significantly improves the accuracy of CSPH diagnosis compared to existing methods like the ANTICIPATE±NASH model.
