Brookdale's HealthPlus program, the company's value-based care coordination program, doubled the number of communities it served this year and led to positive outcomes across operations and resident care. The program resulted in 80% fewer emergency department visits and urgent care visits, 66% fewer hospitalizations, and 61% higher annual wellness visit completion rates.
