Penile prostheses have revolutionized the management of erectile dysfunction; nevertheless, like any other medical device, they come with complications. Prosthesis erosion is a relatively common and catastrophic adverse event associated with these devices. While most guidelines recommend the prompt removal of eroded prostheses, re-implantation and close observation may be considered in patients who show no signs of sepsis, local infection, or necrosis during preoperative evaluations and surgical exploration. There are several therapeutical strategies for pectus excavatum deformities. For mild cases without cardiological neither respiratory impact, in Hospital General Universitario Dr. Balmis Aliacante we recommend conservative treatment, vaccuum bell therapy or 3D customized prosthesis for soft tissue repair, depending on the patient’s preference. This afternoon Dr Francisco Lirio and I performed 2 cases of mild and moderate pectus excavatum with 3D customized silicone prosthesis. This is a simple technique, with a postoperative less painful, smoother and faster than with conventional surgical techniques consisting on remodelation of bony chest wall. Postoperative recommendations are essential to let the body get used to the foreign body of the prosthesis while avoiding complications such as seroma or infection. Cosmetic outcomes are excellent, and we should collect data to assess its long-term outcomes. ACV.EXTERNAL

SPREADING SURGICAL KNOWLEDGE.8 Duodenal Perforation: How to.manage. When facing with this complication, I accomplished a 4-STEPS surgical treatment as follow: - FIRST STEP direct repair of duodenum with not adsorbable stiches (3/0 prolene) reiforced by suturing jejuneal wall over the duodenal perforation after smooth electrocautery application on the serosa of two viscera. This manouvre produces faster adhesions between the two viscera, with good protection of duodenal wall suture; - SECOND STEP biliopancreatic juice external diversion (Rodney-Smith procedure or percutaneous drainage) on continuous aspiration; - THIRD STEP total duodenal exclusion from gastric content transit, by means of uncut suture on pylorus. Gastrointestinal transit is assured with a gastro-jejuneal anastomosis, with a NG tube positioned in the afferent jejuneal loop tu avoid overdistension of sutured duodenum until peristalsys recovered; - FOURTH drainage of periduodenal space with one coaxial tube or two tubes for continuous flushing with saline solution at 20-40 ml/h to dilute and wash out residual enzymes present in the area. Due to this strategy I had excellent results in most patients treated. I advise this approach for best outcome.
