An 84-year-old patient with severe primary mitral regurgitation and prohibitive surgical risk was recently treated successfully with the Pascal ACE device at Baptist Health South Florida. The patient presented with a large flail P2 segment (flail gap 10mm, flail width 25mm). The procedure involved implanting three Pascal ACE devices, minimally invasive transcatheter edge to edge repair. This resulted in a remarkable reduction of mitral regurgitation from torrential to trace to mild and decreased the V wave from 60mmHg to 10mmHg, demonstrating the device's effectiveness in addressing complex mitral valve pathologies. The patient was discharged the day after the procedure. Here are my key takeaways on the Pascal ACE device's advantages: 1. Smooth guide catheter/dilator assembly: Facilitates smoother insertion through vascular access and traversing the septum (Potentially reducing the need for balloon septostomy in difficult to cross septum with MitraClip guide). ✅ 2. Easy maneuverability in the left atrium: Allows for atraumatic navigation and precise device placement. 👌 3. One-to-one transition of implant catheter: Reduces tension built up in the system leading to losing the orientation while crossing the valve. Simplifies the implantation process and enhances procedural efficiency. 🔄 4. No screws to deal with 5. Enhanced navigation under the valve: Easier to navigate and adjust the device's orientation without the risk of leaflet entanglement on the grippers. 🧭 6. Liberal use of independent grasping: Enables secure leaflet capture with a lower risk of leaflet tear due to the horizontal rows of retention elements. 💪 7. 📜 Exciting News Alert! 🖋️ Governor Kristi Noem officially signed South Dakota House Bill 1099 into law on March 4th. 💉 This ground-breaking legislation grants optometrists expanded authority allowing them to administer specific injections and perform laser procedures. Optometrists are now empowered to conduct three key injection procedures: Botox for medicinal use, steroid injections for chalazion treatment, and anaesthetic injections for removing pedunculated skin tags. Additionally, they can perform two essential laser procedures – selective laser trabeculoplasty and YAG laser – along with providing intense pulsed light therapy for dry eye. 🏥 On another front, NHS England reports an increasing number of patients being taken off the reported waiting list, with 3911 instances recorded between December 2022 and November 2023. This action follows guidance introduced in October 2022, allowing clinicians to remove patients from the NHS elective waiting list after they have declined two offers of treatment and place them on ‘active monitoring’. This approach is deemed appropriate in cases where patients may need to address factors like weight loss or smoking cessation before undergoing surgery or reach a certain age. However, inappropriate use of this protocol includes scenarios where elderly ophthalmology patients prefer not to travel or due to capacity issues or long MRI waiting lists. 🤔 What are your thoughts on these developments? The exact prevalence in children is not reported, but they are thought to occur more frequently in younger individuals when compared to adults. The age old treatment for mucocele involves surgical excision, which is associated with mental trauma and discomfort to the patient. Other treatment options that can be considered include cryosurgery, CO2 laser ablation, micro marsupialization, intralesional corticosteroid injection, marsupialization and electrocautery. Cryosurgery is the procedure where there is deliberate destruction of tissue by freezing, using liquid nitrogen. This treatment is well received by patients, as minimal or no local anesthesia is used, relative lack of discomfort, no bleeding and minimal to no scarring after healing. #quality hub Aldrete's scoring system(also known as Post Anesthesia Recovery Score (PAR-Score) ** is a commonly used scale for determining when post surgical patients can be safely discharged from the post-anesthesia care unit (PACU), generally to a second stage (phase II) recovery area, hospital ward, or home. **The Aldrete scoring system evaluates five key parameters: 1. Activity: Ability to move all four extremities voluntarily or on command. 2. Respiration: Ability to breathe deeply and cough freely. 3. Circulation: Blood pressure and heart rate within 20% of the pre-anesthetic level. 4. Consciousness: Fully awake, oriented, and responding to verbal commands. 5. Oxygen saturation: Oxygen saturation of 92% or greater on room air or the patient's baseline oxygen requirement. **Physician approval: - The final decision to discharge a patient from the PACU is typically made by the attending physician, who will review the patient's Aldrete score and overall clinical condition. - The physician may also consider additional factors, such as the type of surgery or procedure, the patient's medical history, and any special considerations, before approving the discharge https://lnkd.in/d4T7wTcP #quality hub

💧CSF LEAK💧 The other day when I was in the ED working triage, a man checked in complaining of a headache. He recently had a brain tumor removed and was experiencing some swelling around his incision and feeling off balance. These complaints automatically place this man as a high acuity on our triage scale. As we are talking and I’m assessing his scar and the very obvious swelling, I asked him if he had noticed any new drainage from his ears or nose recently. His response was, ‘actually, whenever I bend over forward, I have clear fluid pouring out of my nose’. 🚩RED FLAG 🚩 This is something that can happen with a cerebral spinal fluid [CSF] leak. Your CSF is the fluid in your spine and brain. When the layer surrounding the brain and spine called the dura is disrupted, a CSF can leak can happen. Leaks can be a result from a traumatic injury to the head or spine, a lumbar puncture, epidural, or post brain surgery-just to name a few. This is a medical emergency as a leak can cause serious infection, swelling and or bleeding in the brain. A leak can be diagnosed by a beta transferrin test. This tests for CSF in the blood-which should normally not be present. A CT and MRI is also used to confirm diagnosis. Definitive treatment varies based off of what caused the leak, but can include just observation to requiring surgery. Fun fact: did you know that when CSF is absorbed into gauze or tissue, it will leave two distinct rings behind? Over the 2022-23 academic year, our senior design team collaborated with medical professionals to address unmet clinical needs. CRNA Jane Swing, from the Medical University of South Carolina (MUSC), identified a critical issue in monitored anesthesia care (MAC) cases. Hypoxemia affects over 3 million surgical patients annually, highlighting the need for innovative solutions. Enter AnesthEZ, a foam pillow with a rigid set of arms designed to enhance patient oxygen saturation levels during surgery by proactively positioning the neck and mandible. This design prevents hypoxemia and aims to reduce postoperative complication rates, ultimately improving patient outcomes. AnesthEZ's adjustable mandible and jaw holders offer a practical and effective approach compared to manual methods, potentially revolutionizing airway patency maintenance in clinical settings.
