A new study from the American Medical Association has found primary care physicians (PCPs) now need 27 hours a day to complete their clinical and nonclinical tasks — thanks in large part to the ever-growing burden of electronic health records (EHRs). While this trend is certainly not "new" news, it does reinforce the argument that more work is required at an IT level to make the EHRs easier and efficient for clinicians to use. The research also found PCPs were inundated with several types of EHR-related responsibilities, including more medical advice requests (+55.5%) and more prescription messages (+19.5%) per 8 hours of scheduled clinic appointments. Based on just over 1,000 responses. Interesting but not surprising. Yes, these Medscape survey methodologies may not be flawless, but they provide a reasonable snapshot of clinicians' thinking. Most respondents felt that the RVU methodology was unfair and were unhappy with how their workplace used RVUs. Key Takeaways: 1. The solution is not improved workflow as there is only so much AI and other healthcare professionals can do or seminars to reduce Burnout but to eliminate the documentation/ EMR burdens, prior authorizations, improve reimbursements for services rendered with time spent on patient care and not just procedures, reasonable patient panel sizes with adequate time for patient visits. It’s time to face the Big Elephant in the room instead of trying to work around it. The study, spanning from May 2019 to March 2023, paints a stark picture, indicating that PCPs now require an implausible 27 hours a day to navigate their clinical and nonclinical tasks, largely due to the mounting challenges posed by EHRs. Published on January 22, 2024, in the Annals of Family Medicine, the study highlights a significant surge in EHR time, particularly during the pandemic, impacting both professional and personal hours ("pajama time"). KEY IMPARITIVES FOR CHANGE: 💙Total EHR time must be curtailed, with a targeted approach to reduce the 28.4% increase observed. 💙Strategic initiatives to address the staggering 58.9% rise in orders and the 24.4% increase in inbox engagement. 💙Immediate action to counteract the 19.9% surge in EHR time on days without scheduled appointments. The digital workload has led to a concerning reduction in PCPs' clinical hours, exacerbating the shortage of primary care physicians and compromising patient access to quality care. Hospital Employed Groups Need to Accept Risk to Achieve Financial Self-Sufficiency It is exceedingly difficult for hospital employed groups to achieve financial self-sufficiency - especially with multiple primary care providers (PCP’s). Family practitioners, internal medicine and pediatricians are seeing salaries of $250,000 with potential increases. There is more need for primary care providers than supply which will continue to drive up salaries faster than reimbursement. The loss on hospital employed primary care physicians typically exceeds $200,000 and this gap decreases by improving productivity and seeing more patients. However, PCP’s can increase their revenue by assuming risk and helping to avoid unnecessary emergency room visits and hospital admissions. Enrolling in value-based contracts provides primary care physicians with an enhanced revenue stream and the ability to increase compensation. Practices have shown success by establishing systems to monitor and control patients with co-morbidities such as diabetes and cardiovascular issues and actively monitor patients’ use of health resources. Over 50% of all Medicare patients are in health advantage programs. Primary care providers need to develop systems to manage the care and cost provided to this important patient populations. The US is projected to face a shortage of 64,000 physicians this year, increasing to 86,000 by 2036. The reasons? Aging doctors, increased burnout, and skyrocketing patient demand. Nearly 1 in 3 doctors are likely to leave their practice within five years. Pharmacists are uniquely positioned to help fill gaps amid the growing primary care shortage. They can effectively manage medications, conduct health screenings, and provide patient education.
