How committed are you when it comes to using evidence in decision making? If we're honest, I suspect most of us are fully committed when we like the results; somewhat less committed when we don't... "Patient experience surveys should be viewed as early warning systems. The scores may tell us where we are doing well and where we have room for improvement, but they do not tell us how to fix the problems patients identify in their responses. After receiving their HCAHPS results, many people ask me if they should do another survey to identify more detailed intervention strategies. The answer is ‘no’! Hey everyone, there's a mindset shift we need to talk about - it's about preventing hospital readmissions. We're so often entrenched in the belief that readmissions can't be avoided, it's time to make a change. I've coached numerous health administrators to navigate this. Here's what's required: 1/ Quit clinging to the notion that readmissions are inevitable. 2/ Concentrate on the blueprint of proactive patient care before focusing on the reactive strategies. 3/ Craft a systemized preventative approach, a discharge plan you can measure.
