Hypertension is a leading medical problem in the US, and except in the very extremes, it is asymptomatic. It is a disease with a specific finding. The medical term is pathognomic finding, which is an elevated blood pressure most often found during a physical examination. In the most common mechanism, a stiffening of the arteries because of atherosclerosis, makes the heart, also at risk for atherosclerosis, now termed coronary artery disease, work harder to generate these higher pressures, putting it at greater risk of failing. There are other hormonal influences as well as interactions with the kidneys that help regulate blood pressure. 𝐀𝐩𝐩𝐚𝐫𝐞𝐧𝐭 𝐫𝐞𝐬𝐢𝐬𝐭𝐚𝐧𝐭 𝐡𝐲𝐩𝐞𝐫𝐭𝐞𝐧𝐬𝐢𝐨𝐧. Not a common term that everyone is aware of. Apparent resistant hypertension (aRH) is a type of high blood pressure that does not respond to typical anti-hypertensive medications. This is prevalent in 1 in 10 patients with hypertension. A research conducted by Cedars-Sinai's Smidt Heart Institute, LA, California, suggested that the affected group of patients could better manage their aRH when treated with well-managed aRH were more likely to be treated with a medication called mineralocorticoid receptor antagonist (MRA). Professionals urged patients and doctors to consider alternative therapies and treatment for patients with aRH instead of following a medical regime which prescribes four or more antihypertensive medications. Wegovy: Good for Weight Loss and Health or Should We Proceed with Caution? 💉 Wegovy is an FDA-approved injectable medication developed specifically for weight loss. As blood sugar levels rise, this medication helps the pancreas produce more insulin. Insulin then aids in moving blood sugar into the body's cells to be used for energy. So, is that a good thing? 👍 Patients are seeing improved liver function and lower rates of stroke, as well as kidney and heart disease. A forthcoming study is expected to show a 20% reduction in heart attacks, strokes, and deaths from cardiovascular causes. This suggests to me that we are likely to see this medication used for much more than just obesity. However, what are the cons of using this weight loss medication? Yes, when patients discontinue the medication, they may regain the weight, but consider this: what happens when a patient stops taking medication such as Semaglutide for cardiovascular disease? Are they then susceptible to a heart attack? What if a patient discontinues any medicine for diabetes, hypertension, or hypoglycaemia? Do people really take Wegovy for extended periods? In my experience, I have seen individuals describe it as life-changing and state that it has had a positive impact on their lives. They claim they wouldn't have been able to lose weight without it, and they enjoy the accompanying health benefits. However, like anything, there is a risk. Do scientists understand precisely the mechanism by which these medications work on the receptors in the brain, and is the slowing of food through the digestive system a concern? I am aware that many doctors prescribe this weight loss medication. What are your thoughts? 🧐

The STEP trial evaluated the effects of semaglutide (Ozempic or Wegovy) 2.4 mg weekly in participants with heart failure, obesity (BMI ≥30), and type 2 diabetes. Patients received either semaglutide or placebo for 52 weeks. The primary endpoints were changes in Kansas City Cardiomyopathy Questionnaire (KCCQ) scores and body weight. Semaglutide significantly improved KCCQ scores (+14 vs. +6 with placebo) and led to weight loss (-10% vs. -3% with placebo). Secondary outcomes, such as increased 6-minute walk distance and reduced CRP levels, also favored semaglutide.
