Welcoming a Historic Cohort of Physicians in Training! South Texas Health System, located in McAllen, TX, is excited to welcome the first-ever cohort of Family Medicine and Internal Medicine residents as part of the STHS GME Consortium. Last week the 30 residents kicked off a two-week orientation program at STHS Edinburg. The South Texas Health System GME Consortium’s residency programs, accredited by the Accreditation Council for Graduate Medical Education, will teach residents from around the world how to provide evidence-based primary care to patients in our communities through a blend of supervised patient care experiences, clinical teaching, and didactic activities. Following the orientation program, the residents will officially begin their training on Monday, July 1st. #ThisIsUHS #UHSis45

I’m very committed to continuing medical education. Each quarter, I present a workshop for our entire staff and provider community. We shut down our office for half a day and learn together through mock drills, review of standardized processes, and discussions around optimizing the patient experience and improving patient outcomes. This August, our team will come together to share patient histories and learn best practices. These workshops support our mission: to deliver accessible, equitable, comprehensive, and safe preventive care and chronic disease management for our Northwest Indiana community. Last weekend, I also took time to invest in my own continuing medical education by attending the Cleveland Clinic’s 36th Annual Intensive Review of Internal Medicine. During the conference,my determination was to find ways for our provider community to consistently adopt guideline-directed medical therapies for multiple chronic disease management that would control the cost of care by eliminating redundancies and waste, reducing harm to patients and enhancing clinical outcomes. We also are committed to the Choosing Wisely campaign, an initiative of the American Board of Internal Medicine (ABIM) to promote conversations between clinicians and patients to facilitate the above models of care: https://lnkd.in/gZPpbAwM “Choosing Wisely calls upon leading medical specialty societies and other organizations to identify tests or procedures commonly used in their field whose necessity should be questioned and discussed with patients,” according to the ABIM. Tomorrow’s doctors need to appreciate that health is beyond just diagnosis and treatment of specific conditions. An ageing population is more likely to have multiple morbidities (long-term conditions). The challenges of #multimorbidity are that patients present with complex self-care needs; care is provided by several teams often in isolation, resulting in fragmented care and an increased use of resources- including emergency healthcare. Socioeconomic deprivation can add a further layer of complexity while providing health and well-being care for someone with multi-morbidity. Doctors need to consider the role of #socialdeterminantsofhealth while providing care for their patients. Advances in healthcare have helped in the management of individual long-term conditions, thereby adding years to our lives. However, to improve the quality of life in later years, doctors need #Generalism skills to deal with multiple long-term conditions. We need to change the way we train future doctors. Medical education has traditionally been designed around individual specialities (such as medicine, surgery, obstetrics and gynaecology, cardiology, and ophthalmology). It focuses on training future doctors on the pathology, diagnosis, and treatment of individual health conditions. We need to move away from the current model towards developing an #inclusive practitioner for all types of patients and problems. We need to integrate #Generalism into all specialities, not just a few. #IntegratedCareSystems need a workforce with population health management skills to tackle health inequalities and embed prevention into healthcare. It will be great to collaborate with #IntegratedCareSystems to co-design a curriculum which is #sociallyaccountable.

Teaching medical rounds in intensive care units (ICUs) are essential for resident education. However, the ICU’s high workload can hinder these rounds. We propose a new approach that is based on the constructivist theory of learning communities. This approach emphasizes active, collaborative learning through interaction.
