According to a BBC article, hospital bosses affirm that expanded capacity could cut hospital waiting lists.   "Our ambition is to get to a position of having no patients waiting more than 65 weeks by the end of September," said Simon Evans, Chief Operating Officer at the University Hospitals of North Midlands NHS Trust (UHNM). He announced an expansion in their gastroenterology and respiratory services, aiming to eliminate 15-month waits for treatment by the end of September.   An HSJ article shows that NHS England's data reveals that the number of patients waiting over 65 weeks for treatment has risen to over 50,000, ending a six-month decline. The overall waiting list grew by 34,000, reaching nearly 7.6 million, a figure higher than in January 2023, with the number of patients waiting over 18 months also rising above 5,000.   at 18WS we believe that capacity is the cure. Read this today on the outcomes of a cohort of Heart Failure patients managed in a Virtual Ward environment across Central London & West Herts. Very compelling and positive evidence that VW's can work for targeted groups of patients that in turn may help reduce demand on acute hospital bed capacity. The summary paragraph is as follows: This is one of the first reports on HFVH in the UK. Acute LOS was on average 3 days/36% less. There was a trend towards reduced readmissions rates. The 30-day death rate of 1.6% is as expected in this cohort, so there is no signal on safety. There was a high degree of patient acceptability. If these results were replicated in larger cohorts HF VH could result in major savings in terms of IP LOS with no concerns re safety and excellent patient satisfaction. Mass Casualty Respiratory Failure and Mechanical Ventilation: Over the last two decades AARC has partnered with the Centers for Disease Control and Administration for Strategic Preparedness and Response to train clinicians on the role of the respiratory therapist in mass casualty care and the ventilators in the strategic national stockpile. All in anticipation of a mass respiratory failure event. COVID-19 tested this preparation and challenged the worlds health care system. Shortages of staff, PPE, ICU beds and ventilators strained even the most prepared institutions. Hospitals were required to improvise on the fly. Staffing models were created and the use of travelers became routine. The use of PPE amongst shortages placed caregivers at risk, challenging the duty to work paradigm. The role of the respiratory therapist evolved. New treatments were devised, tested and either adopted or abandoned. There were 3 types of ventilators in the stockpile (a total of approximately 14,000 ventilators). During the COVID pandemic, this number ballooned, with the addition of approximately 150,000 ventilators (over a dozen new models- 2 of the 3 original devices were scrapped). The NHS has now rolled out more than 10,000 virtual ward beds to care for patients in their own homes, helping to deliver on a key government priority to cut waiting lists. Virtual wards allow patients to get hospital-level care at home safely and in familiar surroundings closer to family, friends, and carers, helping speed up their recovery while freeing up hospital beds for patients that need them most. More than 240,000 patients have now been treated on virtual wards thanks to the world-leading initiative. Research shows that people who are treated at home recover at the same rate or faster than those treated in hospital. On virtual wards, patients are cared for by clinicians who can provide a range of care, including blood tests, prescribing medication, or administering fluids through an IV drip. They are reviewed daily with a home or virtual visit as part of the ‘ward round’, as well as the use of technology like apps, wearables, and other medical devices so staff can easily check in and monitor their recovery. The introduction of 10,421 virtual ward beds will allow patients to get expert treatment for illnesses such as chronic obstructive pulmonary disease (COPD), heart failure or frailty conditions at home.
