The new series airing on Channel 7 this week (14 Feb, 24) “Mr Bates vs the Post Office” sheds light on a scandal that engulfed one of the UK’s most venerable institutions, the GPO (General Post Office), or more commonly known as the Post Office. It was originally established in 1660 and since then has been a mainstay and essential part of communities across the length and breadth of the UK. One of the most serious problems facing technology is a #businessethics and misaligned incentives problem. Companies have a strong incentive not to acknowledge bugs to clients, especially when they can fix the bugs without the clients knowing. Clients here "bugs" and think that means the tech company screwed up, even though bugs are just an ordinary part of designing and developing software. We need to build a culture (along with a legal infrastructure) that allows for easier disclosure of bugs and more open dialogue between technology companies and their clients. As I wrote at CEPA, the Post Office scandal in the UK goes well beyond that problem, to wildly unethical and illegal business practices by Fujitsu and Post Office leadership, but we should take it as a broader learning experience and take the opportunity to revisit how we incentivize disclosure of bugs and correct errors cased by bugs. https://lnkd.in/e2bgdrAc

In the UK and Ireland, business, administrative and electronic records are generally admissible to evidence in criminal cases, relying on the presumption that the "computer is always right". But Futijsu's Horizon database system wasn't always right, leading to countless ruined lives of totally innocent people. There have been calls in the UK to amend the law to require that evidence being given that the computer system was functioning correctly, and this is echoed here in the Irish Times opinion piece. But what might that look like in practice? Will the state need to bring in an expert for each and every prosecution to attest that the software was behaving as it should?

Not the first time a software bug is affecting human lives - but certainly this is the biggest one. Therac-25 accident looks tiny in comparison! A 2021 article (https://lnkd.in/eCk_t3NF) summarising what we are again reminded of this week - now of course making national headlines in the UK and in the British parliament. Tax payers will pitch in about 1.4 billion GBP in compensation! According to Computer Weekly's 2021 article "...Horizon’s Epos system was initially built with “no design documents, no test documents, no peer reviews, no code reviews, no coding standards”. Too much reliance in the hands of unverified technology can and is affecting human lives. Today it is software, tomorrow it will be hardware, or a combination. Both software and hardware design needs specification, and rigorous test & verification. My understanding is that even basic testing was not carried out in this case never mind the use of more advanced verification. #Formalverification is one of the ways of enforcing rigour - however formal is not a replacement for poor validation although it can in many cases identify validation issues as well.

The Post Office scandal TV series is painful to watch. It’s painful because it is blatantly obvious that people’s belief in technology was so loud that it silenced their ability to listen to people. And this caused catastrophic consequences. The saying ‘computer says no’ doesn’t even touch the sides where this scandal is concerned. People were ignored, passed off, and dismissed by leaders who led with zero compassion. This is a historic moment in time. But for many people affected it is too little too late. Technology will never replace people. AI can take way the headaches of admin, but people are integral. People are the heart of your business, and listening to them is crucial.
