Nearly 40% of American have a chronic disease, and the number of Americans age 65 and older grows larger every day. At University Hospitals, we’re navigating these challenging waters by looking to our primary care providers. Our new Systems of Excellence initiative relies on their expertise and diverse divisions across our health system to help our patients more successfully control damaging chronic diseases and get the preventive screenings that are so essential to good health. Check out the latest blog from UH CEO Cliff A. Megerian, MD, FACS, Jane and Henry Meyer Chief Executive Officer Distinguished Chair, to learn more about our efforts and why we see this important initiative as nothing short of reimagining primary care. #healthcareleadership #systemsofexcellence #primarycare #chronicdisease #preventivescreenings
