Habib Jabagi MD, BSC, MSc, MLP, FRCSC, cardiovascular surgeon, recently spoke at The Houston Aortic Symposium “Frontiers in Cardiovascular Diseases.” This conference invites cardiac surgeons, cardiologists, vascular surgeons, and other healthcare professionals involved in the care and treatment of patients with aortic diseases to discuss diagnosis and treatment paradigms for aortic diseases. Dr. Jabagi presented a case report titled, “Challenging Complications in TEVAR: A Case Report of Contained Aortic Rupture, Intraluminal Graft Thrombus, and Endoleaks,” discussing the complications that may result from thoracic endovascular aortic repair (TEVAR), a minimally invasive procedure used to repair a thoracic aortic aneurysm or dissection, and how to properly address them, which sometimes require complex redo open repairs like this case. #Cardiology #CardiacResearch #CardiacSurgery #MedicalResearch

Monitoring #cardiothoracicsurgery to reduce pneumonia. Here are some highlights: From the discussion: Our study also revealed that IONM reduced the incidence of pneumonia. Postoperative pneumonia is the most common complication after esophagectomy, with an incidence of up to 60%. Several recent studies showed that RLNP is associated with pneumonia, and multivariate analysis showed that RLNP is the primary independent risk factor for pneumonia. (RR = 2.3–6.2). Our study supports the previous studies that IONM can reduce pneumonia by preventing RLNP. Although inconsistent with a previous meta-analysis, our results are more reliable due to the larger number of studies included and insignificant heterogeneity. A study suggests that RLNP is an independent prognostic factor for pneumonia and POHS. In addition, we found that IONM does not increase the operative time and the risk of other common complications, which reflected the feasibility of IONM. From the conclusion: Our meta-analysis demonstrated that the application of IONM during esophagectomy reduced the incidence of RLNP and pneumonia and was associated with more mediastinal and total LN dissected. In addition, IONM does not increase the incidence of aspiration, chylothorax, and anastomotic leakage and does not increase the total operative time or postoperative hospital stays. A research study co-authored by members of Valley's cardiothoracic surgery and interventional cardiology teams was recently published in Reviews in Cardiovascular Medicine. The authors aimed to describe and compare the characteristics of patients who receive the Sentinel Cerebral Protection System (CPS) - a device developed to reduce the risk of embolic strokes associated with debris produced during TAVR - with those who do not in a “real-world” setting of consecutive transcatheter aortic valve replacement (TAVR) patients, and evaluate its impact on postoperative complications, namely stroke. The study demonstrates that in a “real-world” setting, the Sentinel CPS device is more likely to be used in healthier and less complex patients. In analyses adjusted for illness severity and patient complexity, CPS use did not have a significant effect on the incidence of in-hospital/30-day stroke or the composite endpoint of death, stroke, and bleeding/vascular events at one-year. Clinical findings were central cyanosis, a heart rate of 120 bpm, a blood pressure of 100/70 mmHg and a decreased peripheral oxygen saturation of 89%.
