For some, the need for home care is the result of an unexpected health emergency. For others, it evolves over time as it becomes more difficult to perform everyday activities. However the need arises, it can be difficult to broach the topic with a loved one. Here are some tips for starting the conversation, from our blog: https://smpl.is/8vfyv

Just one Signify Health in-home evaluation can reveal a comprehensive view about a person’s health, which is why Signify is an important piece of our vision for a connected health care ecosystem. Our clinicians spend about 2.5x longer with a member than at a traditional primary care visit and can see how the home environment impacts a person’s health. Social determinants of health like food insecurity, safety hazards and more can play a major role in health outcomes, which is why our clinicians can help close these gaps as part of their visit. The intended beneficiaries of health policies, programmes and services are seldomly involved in the design, delivery and or review of care, missing out on contextual perspectives and or opportunities Given the information asymmetry in healthcare this can result in multiple unintended consequences which include poor program uptake, suboptimal health outcomes or cost inefficiencies among others Equity and context are key principles in global health and we must place social participation for health at the centre of our health policy and programming #PeopleCentredDevelopment 4 #SustainableDevelopmemt

📎Capturing the lived experiences, needs and views of citizens and communities is key to shape responsive and resilient healthcare policies, systems and services. FACT 1️⃣: There is not a one size fits all approach when it comes to involving patients and the public in healthcare research or service delivery. It can, for instance, depend on the community, on the goal of the consultation or on the scale of the project. FACT 2️⃣: Convert stories into actions! If you are inviting your community to speak up, you need to ensure you are collecting data that can be transformed into actions. Simply put collect data that can bring about positive change. FACT 3️⃣ : Be inclusive and challenge the concept of 'hard to reach' groups and audiences. The broader and more diverse the perspectives you collect, the more equitable, out of the box and effective solutions you will identify. As in many other topics, #RareDiseases can offer a lot of insight and serve as an informative model 😉 For more facts, tips and advice on how you can collect actionable data and keep your community engaged, explore: 📢 Speak up.
