40% of US physicians aren’t required to participate in MOC programs because of grandfathering. But, besides the unfairness of this decision, it also revealed something far more troubling. Karen Schatten, Associate Director of the National Board of Physicians and Surgeons shared with us that she often talks to physicians, old and young, who agree on one thing: They are no better (or worse) off than their counterparts. Younger doctors who take these programs are no better off than the older doctors who aren’t required. This means the time investment pulling them away from their specialties and practices isn’t yielding the benefits they should. Fortunately, there is a potential solution in the works. Proposed 2025 Medicare Physician Fee Schedule: Key Updates The proposed 2025 Medicare Physician Fee Schedule includes several significant changes for physical therapy: 1. Payment Adjustments: A 2.8% decrease in the conversion factor, impacting payment calculations for CPT codes. 2. PTA Supervision: General supervision for PTAs in outpatient settings, aligning with other settings to improve service access. 3. Plan of Care: Easier initial plan of care certification with physician's signed order. 4. Practice Expense Adjustments: Increased values for 16 PM&R CPT codes and decreases for three codes. 5. Telehealth Policies: Continued review of provisional PT codes and specific conditions for audio-only telehealth. 6. Quality Payment Program (QPP): Minimal changes, with new MIPS quality measures related to musculoskeletal care and added reweighting options.
