I totally agree. It seems at times that with the nursing shortage in our health care systems, that it is becoming a “fill a seat” or in this case, fill a position whether the appointed nurse has the experience, knowledge or ability or not. Often it is explained away as the new staff member will be “up skilled” which is good in theory, but it just places more pressure on existing staff that are already overworked, over stressed and time poor to do the current excessive work, let alone train others. We need to value and support our current workforce, giving them better resources and time for succession planning and training. Perhaps if the current staff felt heard and supported, less would consider leaving the profession. The only way to improve our health system is to truly value it, value the work and the workers (nurses), and abide by legislative and safety/quality standards to promote and provide best practice. It is not to undermine them or compromise them with employing untrained or unsafe staff and practices, which seems to be the “quick fix” which is actually unsafe and counterproductive.

There seems to be a pervasive lack of education surrounding professional license rules & regulations, particularly concerning the scope of practice. This is a matter of utmost importance, as these regulations ALWAYS SUPERSEDE hospital policies & job descriptions. Ex. Tx position statement 15.14….”Establishes that a nurse has a responsibility and duty to a patient to provide and coordinate the delivery of safe, effective nursing care, through the NPA and Board Rules. This duty supersedes any facility policy or physician order (1983).” - THAT'S A DIRECT QUOTE. As #healthcareleaders, you have a duty to ensure that job descriptions, hiring practices, & assigned tasks align with the scope of practice dictated by the governing body allowing the provider the privileges to practice, whether it be the Board of Nursing or another regulatory authority. It's not enough for organizations to set policies; they must ensure that these policies are in line with legal and professional standards that dictate the practice of the provider. I know this seems pretty logical but, unfortunately millions of organizations across the US have policies and job descriptions that do not align with scopes. This leads me to the discussions of "team #nursing" initiatives, which, in some cases, may lead to #LPNs taking on responsibilities beyond their scope of practice and the overseeing #RNs placing themselves at risk due to organizations placing #LPNs in roles that are not within their scope i.e. #TRIAGE (yes, you did read that right placing LPN's in triage as primary caretakers when they cannot do a comprehensive assessment). It's essential to clarify that it's the #RN who ultimately bears the responsibility for overseeing #LPNs in most instances. #Healthcareleaders and #physicians need to realize nurses do not practice "under" an MD. NO. Nurses are not medical assistants and radiologist technologist and or respiratory therapist that DO have to be supervised by the MD. The RN does not have to be supervised BUT the LPN does have to be supervised and that's by the RN. It's crucial to understand that ignorance is never a defense when it comes to adhering to scope of practice regulations. No one can stand in front of a Board of Nursing and claim they didn't know; it's our duty as healthcare providers holding professional licenses to know and adhere to these rules and regulations that provide us with the PRIVILEDGE to practice. It is literally number one of the standards of practice to know and adhere to them, how can one claim naivety? Let's please ensure that #nurses are not being placed in situations that DO NOT align with their scope of practice and not risking their own professional license and or civil or criminal litigation. As a prior investigator and legal nurse these are realities.

This is such an important point. Nurses, it is your responsibility to understand your scope of practice and be prepared to stand your ground when asked to practice outside of it. Unfortunately, a lot of our hospital systems are pushing nurses, LPNs, CNAs to work harder and harder with less and less resources. This can put you in impossible situations, where you have to leverage skirting legal boundaries with meeting the standard of care. Practicing outside your scope will never hold up in court.The organization that put you in this place will never have your back and losing your license could just be the tip of what can happen. “More than 1/2 of nurses are likely to switch jobs this year.“ That’s what the headline of this article reads. But where does the “research” come from? A study done by a temporary staffing company ( registry/ locums). Locums nurses and registery staff are always changing jobs. That’s the name of the game. I did registry nursing & locums for most of my career. Every 3-6 months I usually had a new assignment. So, this article is extremely misleading since the study was done by a company employing temporary nurses who usually change “job assignments” pretty frequently. I personally work with 2 medical assistants who are currently in #nursingschool. It was incredibly difficult for both to get into a nursing school since they have long waitlists. An interesting perspective on the #nursing #workforce. Though more nurses will be coming in, #bedside nursing might still suffer as it is more of a layover in a career. We still need to address the causes that lead to rates of #jobsatisfaction, #anxiety, #depression to promote this side of the profession.
