Welcoming patients to the era of digitalization, clinics send forms or links via text or email before the scheduled appointment. While this approach promises convenience and efficiency, it's crucial to acknowledge that a significant portion of the patient demographic, particularly in various clinics, is aged 65 and above, often resistant to digital changes. To bridge this gap, clinics prioritize proper staff education and engagement with the new digital system. Patient communication plays a pivotal role, with the team explaining the process over the phone and detailing why this transition is beneficial – meeting insurance requirements and ensuring accurate information. Recognizing that not all patients may feel comfortable navigating this digital landscape alone, clinic staff collaborates with patients and, if necessary, involves family members, caregivers, or friends in the process. "What is the point of all this tech in the NHS, if all you do is chase numbers and don't listen to me?" 🏥 A patient said this to me yesterday whilst we chatted about their blood pressure in a review. And it really resonated... This was an engaged patient who had been submitting BP readings online (so they went straight into the EMR), on medication, exercising and active. But the readings were 2-4mmhg above our nationally set KPIs. So like most practices at the end of the financial year, we were focusing on their numbers to get them in target and meet our contractural requirements, rather than looking at the holistic picture. And guess what, they were frustrated (& so was I!) 🤖 It highlighted to me, that great innovations in healthcare are one thing but they are nothing without considering the #patientjourney and outcome. All the joined up communications, going paperless, using AI or enhanced app functionality is nothing if we don't create solutions with the patient at the centre of it and examine the real world impact of them. Obviously the wider question on how me measure #outcomes in primary care is another issue but that one may be a change too far out of reach for now. 💬 How did we end things? Compromising and working together to create a plan that avoided over medication but supported improved blood pressure and monitoring. As much as I love innovation sometimes there is no substitute for the human interactions that enhance healthcare. So finally a big shout out to our clinicians working hard in primary care at this time of year to get it all done and do the best they can for the patients they see. 🙏 #primarycare #patientcenteredcare #healthtech
