Let’s dream a bit. What if PACE programs (Programs of All-Inclusive Care for the Elderly) were incorporated into all community health centers nationally? These programs better address medical, social, and emotional needs, delivering personalized comprehensive care plans, in a way that appropriately centers culture and equity as well as improved clinical outcomes. The Cherokee Nation’s PACE program is a powerful example of this, among many others. By managing routine care, CHC’s could focus on prevention, wellness, disease management, and maintaining independance, while hospitals could focus on acute and critical clients. PACE programs would also offer culturally appropriate social activities, transportation, and caregiver support, reducing social isolation. This national integration would revolutionize elderly care, promote well-being for seniors by increasing the resources available to support them in their home communities, actively celebrate the cultures represented within the community, alleviate the burden on healthcare facilities and family caregivers, showcase the efficacy of CHC’s (as well as strengthen and diversify their revenue streams, making them more sustainable), and better advocate for the wisdom-holders and culture-keepers of the community.
