We don't have that drug in the store!!! How many times have we heard this after it has been prescribed and you are at the collection site. What do you end up doing? Give the relative the prescription? Call the suppliers to deliver? Is the LPO ready? How do we end up here? How can we prevent this from happening? The third consideration when talking about medication is the selection and procurement. There should be a method of overseeing the hospitals medication list including: 🏵 How listed drugs are used. 🏵 Ensure enough stock is available. 🏵 Have a process to provide medication not normally stocked or unavailable. To achieve good clinical outcomes, whether the hospital is for Primary Healthcare or a referral Tertiary hospital its always advisable to have a list of the medications available. This is what is referred to as a formulary. This list leads into the development of stock levels for pharmaceutical products. So what are some of the questions guiding those entrusted with this function of selection and procurement? 🍀 Who creates, maintains and monitors this medication list (formulary) in the institution? Does it have a committee constituted of healthcare professionals those involved in ordering, dispensing, administering and monitoring medication in the hospital. 🍀 How do you add new medications into the list?? Do we have a mechanism?? 🍀 How do we monitor the prescriptions of drugs to identify its appropriateness? 🍀 How do we monitor adverse events or conditions associated with the use of the drugs during its introductory phase? 🍀 How often do we review the formulary list? 🍀 What is the process of procuring drugs that are not normally stocked or are not included in the formulary? 🍀 What is the process of getting drugs when the pharmacy is closed/off working hours?? LVP stands for Large Volume Parenterals, which are sterile solutions intended for intravenous administration in a volume of more than 100 mL. According to the International Council for Harmonisation (ICH) Q1F guidelines on Stability Testing of Pharmaceutical Products and the World Health Organization Technical Report Series (WHO-TRS) recommendations, the recommended storage conditions for LVPs are: Long-term storage: 25°C ± 2°C / 60% RH ± 5% RH or 30°C ± 2°C / 65% RH ± 5% RH.expand_more Accelerated storage: 40°C ± 2°C / 75% RH ± 5% RH. The product is then tested for its potency, sterility, and other important characteristics.
