Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.

Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. The Oregon Health Authority FY 2022 Hospital Community Benefit Report shows that not only did Oregon’s hospitals continue to invest in their communities in 2022, those investments came during very challenging financial conditions. Rising expenses, workforce shortages, and stalled revenue combined to crater hospital stability. Despite financial headwinds, in FY 2022 community benefit spending rose nearly 20% statewide, and 92% of Oregon hospitals met their community benefit spending obligations, which shows hospitals’ commitment to the communities they serve. #orleg #orpol

Yes healthcare providers partnering with the family caregiver (carer, care-partner, unpaid caregiver, designated support person, informal caregiver) is good for everyone! Family caregivers became increasingly engaged and empowered in their roles. At present there is no system for specifying the work assignment that is being passed from health providers to families who take up the role and responsibility of providing in-home caregiving for family members who can no longer fend for themselves. In the moment of health crisis, families are taking on the work of family care responders to ensure the health and well being of their family member, including receiving medical instructions from the health provider. There is no specific planning for traceable work goals, measurable outcomes, or accountability. There is no organization for reporting results, acknowledging milestones, revising plans to adjust to changing conditions, or formally closing out the family caregiving service when it is no longer needed. We are turning private family homes into extensions of healthcare delivery service without any organizational or operational frameworks or infrastructure to ensure the health and wellbeing of the family caregiving resource. We need families to prepare for family caregiving the way they prepare for fires, earthquakes and floods - the need for a family to turn into a team of family care responders is predictable and can have a catastrophic impact on the health and well-being of families.
