Since 1999, over 600,000 deaths have occurred because of opioid use and it is predicted that without reform, 1.2 million additional people could die by 2029 due to the misuse of opioids. You may be applying the concept of opioid conversion more often than you realize. Let me point out a few scenarios. Ever had a patient on chronic hydrocodone who’s now kept NPO due to abdominal issues? When you try to match their baseline opioid requirement with IV opioids, opioid conversion is involved. Another common scenario is when a patient experiences acute kidney injury and can no longer safely take their chronic morphine. You’ll need opioid conversion to switch to a safer alternative. Or in surgical cases, when you evaluate the total IV and oral opioid use to create a tapering plan, summing up the overall opioid dose again involves opioid conversion. In all three cases, you instinctively know that a new opioid and a new dose are required—and that’s your moment of opioid conversion. ——————— 💊 Daily Pain Dose: Opioid conversion does involve some guessing.
