𝐔𝐧𝐝𝐞𝐫𝐬𝐭𝐚𝐧𝐝𝐢𝐧𝐠 𝐍𝐞𝐚𝐫 𝐌𝐢𝐬𝐬𝐞𝐬: 𝐀 𝐂𝐫𝐮𝐜𝐢𝐚𝐥 𝐂𝐨𝐦𝐩𝐨𝐧𝐞𝐧𝐭 𝐨𝐟 𝐒𝐚𝐟𝐞𝐭𝐲 𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 In healthcare, a near miss (often called a "good catch") is an event that could have resulted in patient harm but did not, thanks to timely intervention. While these incidents may seem minor, they are invaluable opportunities for improvement. By analyzing near misses, healthcare providers can better understand root causes, identify process gaps, and implement changes to prevent future incidents. Here are Seven Examples of Near Misses: 1. 𝐄𝐧𝐯𝐢𝐫𝐨𝐧𝐦𝐞𝐧𝐭𝐚𝐥 𝐀𝐰𝐚𝐫𝐞𝐧𝐞𝐬𝐬: A nurse notices that the hallway floors were recently mopped but without a wet floor caution sign. Recognizing the potential hazard, the nurse quickly places a sign in the hallway to prevent slips and falls. 2. 𝐀𝐭𝐭𝐞𝐧𝐭𝐢𝐨𝐧 𝐭𝐨 𝐃𝐞𝐭𝐚𝐢𝐥: An employee detects ink smears on the label of a specimen, which could have led to a misidentification. She replaces it with a clear, legible label before sending it to the lab. 3. 𝐕𝐢𝐠𝐢𝐥𝐚𝐧𝐜𝐞 𝐢𝐧 𝐒𝐮𝐫𝐠𝐞𝐫𝐲: Before starting a surgical procedure, the team discovers that the incorrect limb was prepped and draped. This near-miss could have led to wrong-site surgery, but swift action prevented a serious error. 4. 𝐏𝐫𝐢𝐯𝐚𝐜𝐲 𝐏𝐫𝐨𝐭𝐞𝐜𝐭𝐢𝐨𝐧: A set of EKG strips containing a patient's private information is left on a worktable. A vigilant LPN notices and conceals it to prevent a potential breach of privacy. 5. 𝐏𝐚𝐭𝐢𝐞𝐧𝐭 𝐒𝐚𝐟𝐞𝐭𝐲: A pair of non-skid socks is left behind in a high-risk patient’s room. Noticing the risk, a nurse alerts a supervisor to prevent possible injury. 6. 𝐌𝐞𝐝𝐢𝐜𝐚𝐭𝐢𝐨𝐧 𝐒𝐚𝐟𝐞𝐭𝐲: A nurse preparing to administer an injection realizes that the medication has expired, averting potential harm by not proceeding with the administration. 7. 𝐃𝐢𝐞𝐭𝐚𝐫𝐲 𝐕𝐢𝐠𝐢𝐥𝐚𝐧𝐜𝐞: A patient care technician identifies that a breakfast tray with solid foods was mistakenly delivered to a patient on a liquid diet. The Power of Customer Attitude, Caring, and Flexibility: A Real-Life Example At our company, we pride ourselves on our unique customer attitude, caring, and flexibility. We go above and beyond to provide the best possible care to our patients and physicians, even in the most challenging situations. 🙌 Let me share with you a story that happened recently that shows how we work as a team to make a difference. 🙌 One of our patients had a serious cardiac event, with multiple pauses in their heart rhythm, lasting up to 12 seconds. Our clinical team tried to reach them and their physician, but they were unavailable. It was Friday afternoon, and the office was closed, with no after-hours number. 😱 That’s when our professional team contacted our local sales representative, who is always ready to help. She managed to get in touch with the medical assistant of the practice, who then contacted the physician, who then called us back. Thanks to the local sales representative’s quick action, we were able to inform the physician about the patient’s condition, and he instructed the patient’s family to take them to the emergency room. The patient received timely treatment, and we were relieved to hear that she was doing well. 😊 This is just one example of how we collaborate across departments to ensure the best outcomes for our patients and physicians. We don’t let any obstacle stop us from delivering the highest quality of care.
