True or false: When a person is told they are living with dementia, they can no longer make their own medical decisions? We call this ability “capacity.” With or without formal testing, doctor’s visits dedicated to discussing capacity should be a routine part of care for many people living with dementia. Caregivers and care partners need to understand what having capacity, or lacking it, means on a practical level for them and their person. Because FALSE: Just because a person is living with dementia DOES NOT mean they lose the ability to speak for themselves and make their own decisions. What I hear is these conversations about capacity are being added on to other visits as an aside. Or not discussed formally at all. The conversation may be too much for a primary care doctor to squeeze into a visit even when they realize how important this is. If a person living with dementia values quality of life, at some point, their goal of care might focus FIRST on comfort. Listen for a few of my thoughts when comfort is the goal and we are thinking about what to do when new medical issues happen. Specifically how to think about the ER and hospital stays. I’m helping clients to think this through because I don’t see the current medical system doing so until stressful decisions need to be made or the person is moving towards end of life. Primary care doctors are struggling to help decision-making caregivers of people living with dementia. I could talk about how and why for hours. There are so many reasons. For one, the US healthcare “system” is broken. It’s a dysfunctional mess. But primary care doctors can still help and positively impact both the person living with dementia and the people around them. I’m here to empower decision makers and care partners of people living with dementia with practical knowledge. To determine whether or not the ER will be helpful for a person living with dementia we need to know… 1 - What is the person’s overall goal of care? 2 - What is the intention of the stay in the hospital? What are we trying to accomplish? 3 - How is the hospital stay going to benefit them? This is how I help clients think through decisions ahead of time inside Make Your Plan with Dr. Lamb. First, I teach them how to identify their person’s overall goal of care. Then I personally help them figure out their person’s biggest risks. Inside they learn the treatment options for these conditions so they can consider whether they fit with their person’s overall goal of care.
