You'll have a lot of information you need to remember because you'll need to use it. Here's the thing with working memory: when dealing with a challenge, the idea is not to rely on your brain! Find a system where you can store this memory and use it quickly. You might be disappointed, whether you're a patient or a caregiver if you try to depend on your memory and brain alone to store the information and access it quickly. We all know what it feels like to be overworked, overscheduled, and juggling multiple demands at once. This familiar, almost relentless state of hustle is, for many, a learned behavior ingrained even before stepping into the demanding world of medical school. By the time high-achieving students reach this stage, they've internalized the belief that pushing through exhaustion—despite the urgings to take breaks and avoid burnout—is the only way to succeed. However, there's a critical truth I am certain of: there are healthier ways to practice medicine. [Full stop.]🛑 While we may not always have the power to change toxic systems individually, we can commit to honoring our own values. When I work with medical students and residents, I help them clarify their core values and commitments. As we move into a new academic year I want to challenge learners to identify one activity that relieves your stress, empowers you, and supports your collective well-being. Whether it's exercising, praying, reading a non-academic book, spending time with friends, sleeping in, turning off your phone, or binge-watching a favorite show, the key is to prioritize this activity unapologetically. In an environment where control feels elusive and failure seems constant—especially when surrounded by equally smart and capable peers—these grounding activities are essential. Remember, the amplification of anxiety often comes from being in unpredictable spaces. To counteract this, we must unlearn the autopilot function that leads to overwhelm and adopt new, healthier ways of caring for ourselves as we journey toward becoming brilliant physicians. Emergent Effects (EEs) refer to complex outcomes or behaviors that arise from the interactions of simpler components within a system. These effects are not predictable solely based on the properties of the individual components but emerge from their collective dynamics. They are seen in complex systems where the whole exhibits properties and behaviors that are greater than the sum of its parts. Understanding EEs requires a holistic perspective, considering how individual components interact and influence one another. The interplay of biomechanics, training load, nutrition, and psychological state creates emergent effects. Small changes in one area can significantly impact overall injury risk and recovery time. You can’t easily address one without considering the others in the care of a patient. Holistic Athlete Care: Integrated care (physical therapy, mental health support, strength training, nutrition) produces emergent effects, leading to better health and performance outcomes than addressing each component separately. We have to be able to embrace complexity. Comprehensive Assessments: Consider physical, psychological, and social factors in assessments. Tailored interventions leveraging emergent effects of multiple aspects improve athletes’ well-being. The world demands so much, we have to be able to acknowledge it all. Multidisciplinary Teams :Collaboration among athletic trainers, physiotherapists, nutritionists, physicians, and psychologists creates emergent effects, resulting in more holistic and effective care for athletes. End of the day and scrolling through my work notes to summarise and reflect on what I’ve gone through with my patients and I reevaluate my role as a lifestyle educator in the AEP field from time to time. More often times than not we think we gotta come up with a “perfect” programme for patients that involves a lot of “cool and new” exercises but this post bypass heart patient put things into perspectives for me. This puts pressure on me to keep upskilling all the time but little did I know I have yet to perfect the basic skills of patient communication and lifestyle coaching at times. Decided to take it slow, sit down with the patient during the consult, take out a piece of paper and do some motivational questioning whilst listing down numbers 1-4 on what the patient knows they need to do and already are currently doing. Within 30 minutes we got a plan in place, and the gentlemen went home with a refreshed confidence that he will now need to do these 4 things as homework. Now it’s not much, but I think having that sort of side by side guidance to help facilitate the changes that people already have instilled in them, would go a long way in my treatment. It’s today that I realised once again, a simple and traditional piece of “paper and pen” can be an effective no-nonsense tool in treatment. Goals of patient: Follow up cardiac rehab ✅ Lung capacity training ✅ Walking capacity training ✅ Stretches to alleviate tightness in sternum and vessel retrieval sites ✅
