These images are of a 13-year-old male presented with a progressive right leg valgus deformity and associated leg length discrepancy of 7mm as a result of a distal femur physeal fracture (Salter-Harris Type 6). Partial lateral physeal arrest was confirmed with an MRI. An acute correction distal femoral osteotomy was performed with varus, extension, and lateral translation. Restoration of normal mechanical axis was confirmed using the Bovie cord test. The osteotomy was augmented with tricortical iliac crest allograft. A simultaneous physeal bar resection was performed. Fixation was accomplished with a 4.5mm Distal Femoral Locking Plate with zero offset and 5° flare from the WishBone Medical PRIMA system. At the 10-week follow-up, the patient had regained full ROM and normal gait without a limp. Lots of interesting orthopedic tidbits today! Our new friend attached to this Xray came in for a second opinion after an unsuccessful PRP procedure for shoulder pain. She did not proceed with their recommendation for a series of two more injections to fix her shoulder looseness. This is her xray on presentation. I looked at it....hmmmm. The history? "Shoulder pops out with every deep breath (literally)". Needless to say the PE and the Beighton score all matched. Moral: #1 PRP probably not the right choice for this shoulder instability. #2 I always need to see the xrays/films and not just a report. #3 I need to be aware of the limitations of PRP/BMC/Adipose so patients can get an truly "informed" consent prior to a procedure. Plan: MRI and likely bony + soft tissue reconstruction of her instability.

Interesting case: 70 years old man. Motorcycle accident at the age of 18: fracture of femur and acetabulum (ORIF), and proximal leg fracture + partial damage to the common fibular nerve. Orthopedic treatment of leg fracture, and evolution towards a "stable" pseudarthrosis. Progressive valgus deformation and destabilization of the pseudarthrosis site which becomes mobile.
