The FDA, in a move that’s as predictable as it is infuriating, has disgracefully chosen not to approve MDMA-assisted therapy for patients suffering from PTSD. Rather than embracing a treatment that could genuinely offer hope, they’ve decided to cling to the same failed strategies that have failed many (including veterans/ trauma victims in high crime locations) for over 25 years. By turning a blind eye to promising new therapies, the FDA has ignored the urgent need for real solutions. Today’s vote is nothing short of a disgrace, a betrayal of those who have already sacrificed so much. The decision demands further investigations into the treatment's safety and effectiveness, delaying potential relief for many veterans. At the root of this decision, as with the UK's stance on decriminalising marijuana, lies the protection of #pharmaceutical revenues. Big Pharma has historically opposed the legalisation and acceptance of alternative treatments that could threaten their market share. In the case of marijuana, #pharmaceutical and #alcohol companies have been known to #bankroll #opposition to its legalisation to protect their interests. #lobbyists Similarly, the hesitation to approve MDMA-assisted therapy may be influenced by the potential disruption it poses to the lucrative market for traditional PTSD medications and treatments. #protections #barriers #moats #headlocks

Trauma-Informed Tuesday (week 2 of a 52-week series): **6 Principles of Trauma-Informed Care** The Six Principles of Trauma-Informed Care In our journey to better support veterans, it’s crucial to understand the core principles that guide trauma-informed care.  These principles ensure that we approach every interaction with empathy, understanding and respect. 1. Safety:  Both physical and emotional safety are paramount.  Veterans need to feel secure in their environment, free from harm or threat. 2. Trustworthiness and Transparency:  Building trust through clear, honest communication helps veterans feel more comfortable and supported in their recovery journey. 3. Peer Support:  Connecting with others who have shared similar experiences fosters a sense of belonging and validation. 4. Collaboration and Mutuality:  Working together as equals, whether between veterans and caregivers or within the broader community, empowers everyone involved. 5. Empowerment, Voice, and Choice:  Encouraging veterans to take an active role in their care by honoring their choices and providing opportunities for self-advocacy. 6. Cultural, Historical, and Gender Issues:  Recognizing and respecting the diverse backgrounds of veterans, including their cultural, historical, and gender-specific experiences.   Why These Principles Matter: By integrating these six principles into our daily interactions and care practices, we can create an environment where veterans feel safe, respected and empowered.  This is a critical foundation for preventing suicide and promoting mental health. As a clinician working directly with Veterans, I've come to realize that most clinicians work with Veterans, whether they are aware of it or not. This awareness has made me sensitive to the importance of additional screenings on intake to understand our clients on a deeper level, even when they may not initially reveal all the facts. The suicide rate among Veterans remains alarmingly high, with over 100,000 Veterans having died by suicide since 2002. It's crucial that we address this epidemic head-on. Using the ASQ screener can serve as a quick and efficient way to initiate discussions about suicide. Establishing rapport and directly asking questions about suicidal thoughts can be even more effective. Surprisingly, individuals are often willing to open up when asked directly, leading to crucial conversations. Research across various modalities supports the effective treatment of suicidal thoughts in Veterans in outpatient settings. Research demonstrates that clients are actually at a higher risk of an attempt if admitted. If providing this level of care feels overwhelming, it's completely understandable. In such cases, please consider referring them to Stop Soldier Suicide, where a trained and licensed team stands ready to provide support using CAMS and BCBT-SP modalities. Remember, asking the right questions, identifying risks, and facilitating immediate help for Veterans is crucial in this process. As an Army Combat Veteran, I've sadly experienced the loss of friends to suicide. My ultimate goal is to prevent such tragedies and support others in getting the necessary help promptly. BILLIONS of dollars are expended on drugs and fruitless psychological and other unproven interventions to try to solve veteran PTSD. Government money is spent on research and unproven, even hurtful drugs like LSD and other psychotropics. Despite all that spending, homeless, incarcerated, depressed veterans and citizens suffer too often with undiagnosed brain wounds. Billions are spent on "mental health" interventions while relapse rates for mental illness are above 50% even without brain wounds. None of this has to happen because there is already a treatment for PTSD and TBI. HBOT (hyperbaric oxygen therapy). The DOD/VA/Army conducted HBOT studies producing data that show this treatment works: it's safe and effective. I completely agree with Juliana, Congressman Dan Crenshaw, Heroic Hearts, and all the others that are speaking out on this. MDMA-assisted therapy shows incredible promise with an 80% remission rate for PTSD. It offers so many veterans hope and a path to regain their health. Unfortunately the other aspect that makes it incredibly attractive, allowing veterans to get off of a lifetime of prescription drugs and lower their healthcare costs, is exactly why ICER and the FDA advisory committee have recommended that they not be approved. These two organizations were completely staffed by people with ties to Big Pharma - the only organization that won't benefit (read loose MILLIONS of $) from MDMA treatment. Please stand for our veterans and for all those with mental health issues. Maybe my bias as a foot and ankle surgeon but I don’t love the idea of reaming 10-12mm reamer across the ankle and subtalar joints and placing a nail without joint prep. There are certainly indications, ie frail limited mobility patient with unstable ankle fracture that can’t maintain NWB etc but to me this is relatively uncommon. If you are placing any fixation across a joint without any attempt to fuse the joint it is a matter of when, not if, the hardware will fail. This can lead to very difficult revision scenarios. I would encourage those utilizing this technique to at least consider using an MIS arthrodesis burr to perform at least some type of joint prep. This can be done quickly under fluoro through stab incisions or at most a 1cm incison. This will at least give the patient a chance at fusing the joint. There are multiple companies that carry such burrs but I personally use #stryker MIS arthrodesis burr and then injectable augment (recombinant PDGF) through the stab incision that the burr went in.
