Traditionally, phlebotomy has been used to lower hematocrit levels, aiming to reduce thrombosis risk. But is it the best approach? Josef Prchal, MD, uncovered a pivotal shift in understanding PV with his 2021 study, “Iron Deficiency in Polycythemia Vera Increases HIF Activity and Transcription of Prothrombotic Genes.” His research revealed that the real concern might lie not in high hematocrit levels but in the phlebotomy process itself. Surprisingly, it's associated with an increased risk of thrombosis in PV patients, potentially due to iron deficiency (ID) induced by repeated phlebotomies. This ID elevates hypoxia-inducible factors (HIFs), which, in turn, may upregulate prothrombotic genes. This study not only challenges existing practices but also opens new avenues for managing PV, emphasizing the importance of careful consideration of phlebotomy's role. It highlights the critical need for ongoing research to refine treatment strategies. He furthered this study on iron deficiency in PV patients with MPN Research Foundation 2021-2023 MPN Challenge™ grant.
