CHF recently convened a consumer panel for Council for Connected Care's "Older People and Aged Care" meeting. The panel, developed in partnership with the Australian Digital Health Agency shared their insights into navigating aged care and healthcare systems.      Consumers Pip Brennan (the panel facilitator), Aunty Cheryl Saunders, Lyn Whiteway, Trevor McMahon, and Dee Ellam, provided the Council with valuable perspectives through their lived experiences in accessing and navigating the aged and healthcare systems, medication safety, trust and confidence with information sharing, and digital inclusion and literacy.     We thank Pip, Aunty Cheryl, Lyn, Trevor and Dee for attending the Council for Connected Care meeting in Sydney this month.

When patients and caregivers were asked to rank the attributes they feel are most essential in creating an Age-Friendly Health System, patients more often said “promotes physical function and independence” is most essential. In contrast, caregivers more often chose “ensures all communications are clear and well understood,” according to a comprehensive consumer survey by WebMD and The John A. Hartford Foundation. Why It Matters? Age-friendly care means recognizing the importance of both perspectives and striving to create a health system that supports physical independence and fosters clear communication. This dual focus ensures that the needs of both patients and caregivers are met, promoting a comprehensive and supportive healthcare environment. Together, we can build a more Age-Friendly future that meets the needs of both patients and caregivers.

Encouraging patients to discuss their end-of-life goals is the best way to ensure they get the healthcare they desire and deserve.
