Affordable Care Act - Trump vs. Harris

This article compares Donald Trump’s and Kamala Harris’s approaches to the Affordable Care Act (ACA) as we approach the 2024 election. It explores Trump’s attempts to repeal the ACA and Harris’s efforts to expand it, particularly through the American Rescue Plan and the Inflation Reduction Act. The discussion touches on mental health and healthcare costs, concluding that neither candidate has yet offered a comprehensive solution to America’s healthcare challenges.

Affordable Care Act: A Comparison of Trump and Harris’s Approaches

When it comes to healthcare policy, the debate surrounding the Affordable Care Act (ACA) remains at the forefront of the 2024 presidential election. Here's a breakdown of key actions and proposals by both Donald Trump and Kamala Harris, providing a fact-based look at their policies.

Donald Trump’s Approach:

  1. Attempted Repeal in 2017: Trump unsuccessfully tried to repeal and replace the ACA. His various plans were projected to leave 51 million Americans uninsured.
  2. Individual Mandate Penalty: Trump deprioritized the enforcement of the individual mandate, eventually reducing the penalty to $0.
  3. Cost-Sharing Subsidies (CSRs): He stopped payments for CSRs, which led to higher premiums but also increased federal subsidies.
  4. Reduced ACA Outreach: Trump reduced funding for outreach efforts, which may have contributed to enrollment stagnation.
  5. Short-Term Plans Expansion: Trump expanded the availability of non-ACA-compliant short-term plans, which restrict coverage for pre-existing conditions.
  6. Enhanced Direct Enrollment: He allowed ACA plan enrollment via online brokers, easing the process for some consumers.
  7. Proposed Block Grants: In budget plans, Trump proposed shifting healthcare funding to states through block grants, which could weaken protections for pre-existing conditions.
  8. Repeal Agenda as a Candidate: Trump has repeatedly stated his intention to "replace Obamacare with much better healthcare." Despite the rhetoric, his specific plans remain unclear.

Kamala Harris’s Approach (Biden-Harris Administration):

  1. American Rescue Plan Act (ARPA): The Biden-Harris administration enacted ARPA, temporarily expanding ACA subsidies and extending them via the Inflation Reduction Act (IRA) through 2025.
  2. Fixing the “Family Glitch”: Dependents of those with unaffordable employer-based coverage can now receive ACA subsidies, closing a gap in the original ACA.
  3. Reversing Trump Policies: Harris helped reverse Trump’s expansion of short-term plans and restored funding for ACA outreach and enrollment.
  4. Record ACA Enrollment: The administration saw record enrollment in ACA plans, thanks to increased subsidies and outreach.
  5. Making Subsidies Permanent: Harris’s healthcare plan seeks to make the expanded ACA subsidies permanent, a significant shift in healthcare policy.

Expanded Insight into Kamala Harris’s Healthcare Plan:

Vice President Harris has been vocal about making healthcare affordable and accessible for all Americans. Some of her key proposals include:

  • Permanent ACA Subsidies: Harris wants to lock in the ACA’s enhanced subsidies, which are already reducing premiums by about $800 annually for millions of Americans.
  • Lower Prescription Drug Prices: Building on the successes of the IRA, Harris aims to extend Medicare’s authority to negotiate drug prices.
  • Maternal Mortality Crisis: Harris has led efforts to combat maternal mortality, successfully extending Medicaid postpartum coverage from two to twelve months in 46 states.

As President, she plans to expand on these efforts, further reducing medical debt and improving healthcare access.

Mental Health and Healthcare Debate: My Takeaways

While both candidates touched on healthcare, there was a glaring omission: mental health. The nation is facing a mental health and addiction crisis, yet neither Trump nor Harris offered significant details on how their plans would address these critical issues.

Kamala Harris’s extension of Medicaid postpartum coverage is a step forward, but it fails to address mental health issues beyond women’s health. Mental health is a crisis affecting all Americans, not just those in postpartum care. I expected to hear more about increasing mental health funding, reducing drug addiction rates, and improving access to care.

On another note, I was disappointed by Harris's focus on Trump’s comments about Senator John McCain. While these moments provide political theater, they do little to move the conversation forward on healthcare, particularly for veterans like myself. Veterans deserve tangible solutions, not jabs at past comments.

Healthcare Costs: Will the Proposals Help?

The cost of prescription drugs continues to be a problem. Harris has promised to expand the $35 cap on insulin and out-of-pocket spending for seniors, but I wonder why similar measures aren’t being extended to all Americans. While Trump has touted removing ACA penalties and redirecting healthcare spending, I am skeptical about how these changes would improve care or reduce costs.

Healthcare Equity: Still Far Off

Neither candidate has laid out a plan that promotes healthcare equity for all. Trump’s plan to divert spending could free up funds, but without clarity on where this money would go, I remain unconvinced. On the other hand, Harris’s insistence that healthcare is a “right” sounds great, but her policy proposals don’t extend far enough to address the broader needs of all Americans.

A wildcard in this conversation is Robert F. Kennedy Jr., whose recent endorsement of Trump raises questions about how his healthcare ideas may influence Trump’s platform. Will we see a merging of their concepts, or will Trump's promises remain vague?

Conclusion

Ultimately, the debate on healthcare is ongoing, with no clear winner in terms of a comprehensive plan that will solve America’s healthcare crisis. Both Trump and Harris offer enhancements to what’s already in place, but neither seems poised to bring about the revolutionary change that many Americans are looking for.

If healthcare is a right, as Harris claims, then more must be done to make that right accessible for all. And if Trump wants to replace Obamacare with "much better healthcare," we need to see a concrete plan with actionable steps.
