Background: The gap in Hospice & Palliative Care (HPC) for end-of-life patients is huge in China. Broken medical systems and fragmented medical services, the advancement of the first regional pilot program of HPC from 2016 is not ideal, and many obstacles at the policy and institutional levels, which restrict the piloting of HPC and the promotion of its experience.

Objective: This study aimed to develop and evaluate "Hospice Palliative Care Initiative/Pyramid" (HPCP) model for national pilot studies in China.

Methods: Obtained the situation of HPC, the factors of the systems, contact methods between various elements and system coordination mechanism through systematic literature analysis. Established HPCP systemmodel for end-of-life patients and stakeholders, guided by theories of Synergetics, Interorganizational Network, Comfort for Outcomes, and adopted integration strategies with TDT(Trans-Disciplinary Team), MDT(Multi-Disciplinary Therapy), Multi-Agency Service Pathway and IDS(Integrated Delivery Systems). Improve and evaluated the application of HPCP model through really visits the law, the interview law, Mitchell score-based approach and Delphi method based on system perspective.

Results: The authors shared the pathway to partner across disciplines, with policy makers, and in research, education and practice with six dimensions. The construction of HPCP integration of HPC systems with Chinese national conditions based on pilot studies in multiple regions, combined with P-design(design based on Product) and G-design(on Geography), will provide a more scientific and effective basis and important reference value to establish the National Hospice Palliative Care system in China, also achieve the optimization of end-of-life medical and social care resources allocation and the improvement of the quality of death.

Conclusions: Based on the continual improvement of two batches of national pilot studies in China that the authors participated in, this pyramid model HPCP, optimize service delivery methods, improve system efficiency, integrate and reshape service systems, and provide a set of sustainable medical and social integration solutions with Chinese characteristics.
