IBM's Commitment to Population Health
Yesterday, the word was flying around the buildings at the HIMSS15 here in Chicago. "IBM decided to acquire Phytel and Explorys."
I see this as a huge win for the nation. IBM Watson Health has already proven it's dedication to healthcare and is now sending a clear message to the nation that health data analytics and population health is how the US will achieve healthier communities.
For the last few years, I was lucky to be a part of the CMS CMMI Patient-centered Medical Neighborhood demonstration project utilizing Phytel. Our communities came together and far exceeded the projects goals. Through close connectivity with other communities across the nation, the demonstration project showed leveraging population health management platforms like Phytel can connect any healthcare delivery entity in the community and provide risk-stratification with actionable interventions across the entire care continuum. We connected skilled nursing facilities, hospitals, private doctors, employed doctors, FQHC's, safety net factilities and many specialty practices...and it worked!
The power in these platforms is to be able to input data from disparate EMR's, assess different populations, identify gaps in care, act on those gaps and demonstrate outcomes down to the patient level. Outcomes are easily visualized on dashboards and on physician scorecards. Having goals and a structured plan to achieve the goals is important. But, having the ability to see progress in near real-time is invaluable.
Clinicians want information. They want to improve the health of their patients. Utilizing these platforms allows them to do so. It also allows clinicians to compare their performance to other clinicians within their groups, states, regions and nationally.
So, when I heard the news, I was immediately energized. We've needed big companies to come to the table to take us to the next level. Now, it's time to scale.
