About the session 

The Health Value Accelerator series aims to bring together population health organizations and leaders to discuss key challenges, learn from experts and strategize on how to successfully implement solutions that will help drive your value-based care efforts for all key stakeholders. 

Who are the organizers?

The Accountable Care Learning Collaborative (ACLC) and CareAsOne have partnered to help population health leaders understand the critical factors that lead to success in value-based care. This educational series will be led by industry experts, and our first interactive workshop will consist of real-world examples. 

What can you learn?

The goal of this workshop is to help participants assess current challenges and work with other population health professionals to build solutions to improve performance. Planned discussion points include: 

• How to design population health models to fit your population health strategy
• How to build or use a strong infrastructure to succeed in risk-sharing agreements
• How to decide whether to build an in-house solution, partner with a vendor to build a customized solution, or buy an out-of-the-box solution

Why is there a need for such a workshop?

• Today’s executives are forced to analyze growing volumes of data to synthesize elusive insights to drive point-of-care impact
• Multiple types of data (e.g., clinical, financial, etc.) and forms (e.g., structured, documents, etc.) add to the complexity
• Disconnected and disparate data sources (e.g., EHR, claims, Aunt Bertha, etc.) create additional gaps and delays
• Successful  approaches and industry best practices are often retained internally, and most practitioners are forced to reinvent the wheel for their teams

Using a personalized,  hands-on format, this session will enable value-minded professionals to get a head start on designing processes and systems that lead to proven success. We’ll also look at ways to evaluate your existing infrastructure to optimize it for greater success. More experienced attendees will also benefit from this opportunity to examine current practices, refine them under the guidance of experts, and build relationships with other leaders on the cutting edge of value-based care.
