A recent article from STAT about the CMS Innovation Center Pilot Program’s End-Stage Renal Disease Treatment Choices Model – based on #ValueBasedCare – provides some great insights and highlights the need for change in the kidney care space. We agree: change needs to happen. Patients living with chronic kidney and end-stage #KidneyDisease are among the most clinically complex and medically underserved. They often face barriers to care and experience other challenges, including transportation needs, housing instability and food insecurity.

At Monogram Health, we believe that physicians and clinicians are the catalysts of change these patient populations need and deserve. Our in-home care delivery model is premised on evidenced-based practices and aligns providers and payers on one goal: improving patient health and decreasing the cost of care.

We take on the financial risk of treating patients living with kidney disease and polychronic conditions and we deliver all the care they need - whether it’s primary care, specialty care, social services, behavioral health supports or transportation. We coordinate with a patient’s existing doctors and whenever possible we engage their family and loved ones in their care.

Our model works. When compared to national averages, our patients experience lowered blood pressure, improved A1C, more in-home dialysis starts, more clinically appropriate transitions of care, and decreased ER and hospital admissions. Improved clinical outcomes translate not only into improved health and quality of life, they also result in decreased costs. We delivered $160 million in savings to our partners last year.
