 Tucson, Arizona, April 10, 2015: For the fourth year in a row, Park Avenue Health & Rehabilitation has been recognized by the Centers for Medicare and Medicaid Services (CMS) and the Arizona Department of Health (AZDHS) as a 5-Star Quality Rated provider of skilled nursing, rehabilitation, long-term care and memory care services.  Along with this top quality distinction, Park Avenue has also achieved a Deficiency-Free Survey Rating from the Arizona Department of Health for a second year in a row.  

Park Avenue has also been recognized by U.S. News and World report as one of the nation’s Best Nursing Homes and was recently presented with the Southland Service Award for demonstrating a firm commitment to assisting, supporting, and encouraging other skilled nursing providers in the community.  

Park Avenue continually strives to be the community’s premiere provider of rehabilitative services, and has proven exceptional outcomes are achievable through teamwork and a commitment to patient-centered care.
