Recognizing When Clients Need Assisted Living: A Guide for Home Care Providers

As a home care provider, it’s crucial to recognize when a client may need, or already be considering, a transition to assisted living. Here's how to identify the signs and refer them to a Senior Living Advisor at A Place for Mom to assist them:

Increased Care Needs:

What It Is: When a client's care requirements exceed what can be provided at home.

How to Identify: Look for signs like frequent medical needs, mobility issues, or significant daily living assistance needs.


Safety Concerns:

What It Is: Situations where the home environment may no longer be safe.

How to Identify: Note any increased risks, such as falls, confusion, or inability to evacuate in emergencies.


Social Isolation:

What It Is: When a client becomes isolated and lacks social interaction.

How to Identify: Observe changes in mood, decreased interest in activities, or lack of regular social contact.


Caregiver Stress:

What It Is: The strain on family caregivers becomes overwhelming.

How to Identify: Look for signs of burnout or stress in family members providing care.

Being proactive in identifying these signs and assisting families in finding the right assisted living facility ensures their continued well-being and safety. As a trusted home care provider, your recommendation to services like A Place for Mom can be invaluable in this transition.

Referring to A Place for Mom:

Home care providers in our network can refer families to an A Place for Mom Senior Living Advisor to help them navigate available communities.

How to Do It: Log in to your Provider Portal and select the option to “Refer to Senior Living”. You can refer anyone in need of assisted living. They don’t have to be a client of your agency.

Home care providers in our network receive a $500 invoice credit for any individual they refer that moves into one our our A Place for Mom partnered communities.
