Recurrent vomiting due to prolonged marijuana use

You may not know that chronically recurrent vomiting can be due to prolonged marijuana use. The Cannabinoid Hyperemesis Syndrome is a subtype of the Cyclic Vomiting Syndrome that occurs in long-term smokers of marijuana. While many patients report that the use of cannabis helps relieve symptoms of nausea and vomiting, chronic and heavy cannabis use has been associated with nausea, vomiting and abdominal pain.

Cannabinoid hyperemesis syndrome is observed predominantly in males who have engaged in daily or near-daily cannabis use for years.

Clinical diagnostic criteria for cannabinoid hyperemesis syndrome:

  1. Episodic vomiting occurring 3 or more times annually.
  2. Duration of cannabis use > 1 year and frequency more than 4 times per week
  3. Resolution of symptoms after a period of abstinence from cannabis.

A few Clinical Management Pearls:

1.        Hot showers improve the symptoms of nausea, vomiting and abdominal pain.

2.        Improvement in symptoms after acute cannabis withdrawal takes typically 1-3 days but symptoms after a period of abstinence from cannabis use will take up to 6 months. As treatment during the prodomal phase of nausea and abdominal pain, triptans, benzodiazepines and antiemetics with NK1 or histamine H1 antagonists can be used.
