  • The SREI Practice retreat On October 20 featured a dynamic morning of insightful talks. We’re still abuzz. Here are some highlights: 𝐃𝐫. 𝐇𝐚𝐤𝐚𝐧 𝐂𝐚𝐤𝐦𝐚𝐤, ““𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭 𝐨𝐟 𝐃𝐎𝐑 𝐚𝐧𝐝 𝐩𝐨𝐨𝐫 𝐫𝐞𝐬𝐩𝐨𝐧𝐝𝐞𝐫𝐬”:” He noted that supplements like DHEA, CoQ10, and growth hormone may improve ovarian stimulation responses, but intraovarian PRP and testosterone have not shown effective increases in pregnancy rates. Strategies such as luteal phase starts and mild stimulation may be appropriate in select cases. Patients should also be aware of potential menstrual cycles during IVF if injectable medications are started in the luteal phase. 𝐃𝐫. 𝐑𝐨𝐛𝐞𝐫𝐭 𝐁𝐫𝐚𝐧𝐧𝐢𝐠𝐚𝐧, “𝐒𝐩𝐞𝐫𝐦 𝐃𝐍𝐀 𝐅𝐫𝐚𝐠𝐦𝐞𝐧𝐭𝐚𝐭𝐢𝐨𝐧 (𝐒𝐃𝐅) 𝐓𝐞𝐬𝐭𝐢𝐧𝐠 & 𝐌𝐚𝐧𝐚𝐠𝐞𝐦𝐞𝐧𝐭”: SDF, referring to breaks in sperm DNA, can lead to lower fertilization rates and recurrent pregnancy loss, often influenced by lifestyle and oxidative stress. Techniques for measuring SDF include the TUNEL assay and flow cytometry, with high SDF linked to reduced pregnancy rates post-IUI and IVF/ICSI. Strategies for SDF reduction include lifestyle changes, antioxidants, shorter abstinence intervals, microfluidic sperm sorting, and appropriate surgical options. SDF analysis is not recommended for initial infertility evaluations. 𝐃𝐫. 𝐊𝐚𝐭𝐞 𝐃𝐞𝐯𝐢𝐧𝐞 addressed the feasibility and funding of prospective and retrospective research in the REI Privademic model. 𝐃𝐫. 𝐉𝐞𝐧𝐧𝐢𝐟𝐞𝐫 𝐊𝐚𝐰𝐰𝐚𝐬𝐬 addressed the history of and upcoming changes to SART outcome reporting, reflecting evolving practice patterns. 𝐃𝐫. 𝐀𝐧𝐠𝐞𝐥𝐚 𝐋𝐚𝐰𝐬𝐨𝐧 highlighted the importance of communication during exams and introduced the VASE technique (Validate, Ask, Support, Empathy) for empathetic care. 𝐃𝐫. How did you first get involved in SREI? "I attended the rite-of-passage SREI Fellows Symposium in 2017. There, I listened to impactful talks and made lifelong professional and personal connections.
