The Vivistim Paired VNS System is the first and only medical device of its kind for stroke therapy approved through the FDA's Breakthrough Devices Program. This journal article highlights the surgical experience from the Vivistim pivotal trial: 🔹 Vagus nerve stimulation (VNS) paired with rehabilitation therapy has been demonstrated to be beneficial in a population of stroke patients. 🔹 Stroke patients have different perioperative considerations for VNS implant when compared to epilepsy patients. 🔹Management of antiplatelet/anticoagulation medications is a critical part of the perioperative treatment plan for stroke patients undergoing VNS implantation. 🔹VNS implantation in a select population of stroke patients can be performed with risk that is comparable to that in epilepsy patients. 🔹 The rate of surgery-related adverse events was lower than previously reported for VNS implantation for epilepsy and depression. 🔹 One participant had vocal cord paresis that eventually resolved. 🔹 There were no serious adverse events related to device stimulation. 🔹 Over 90% of participants were taking antiplatelet drugs (APD) or anticoagulants and no adverse events or serious adverse events were reported as a result of withholding these medications during the perioperative period. Our long-term follow-up study examined whether a long-term home-based intervention using Paired VNS Therapy is feasible and whether the benefits of Paired VNS therapy are maintained beyond 1 year. Here's how the intervention was designed: ⚙️ Participants were implanted with a VNS device ⚙️ 6 weeks of in-clinic therapy with vagus nerve stimulation paired with goal-oriented tasks ⚙️ Home-based rehabilitation with 30-minutes of vagus nerve stimulation activated by swiping a magnet ⚙️ In-clinic and at-home rehabilitation continued for 90 days 🔎 The findings demonstrate that the significant effects of Paired VNS Therapy at the first year were maintained at years 2 and 3, and further improvements in both impairment and function were observed in years 2 and 3. These changes were associated with improvements in measures of activity and participation.
