After you unlocked the Right Field Player Home (spoilers), you might have noticed that, fast travel aside, you have to walk through a lot of interiors to get to it.
This mod adds two special doors buildable at the player home which can be used to enter or leave it from within and from the outside of Diamond City. These doors can be snapped to the walls within the player home, but should work no matter where they are placed. They can also be scrapped again, and should then become buildable next time you enter workshop mode.
Door to The Fens The door can be snapped to the green garage door within the player home, and opens a passage to a similar-looking green garage door on the east outer wall of Diamond City.
Hatch to Diamond City This hatch has four possible snap points on the brick wall below the roof slope, and opens to a place behind Abbot's house. It's a hatch, because there is not enough space for a full-sized door on the inner wall. I just assumed that the ground level is higher within Diamond City than outside.
