The latest MMQ Quarterly Report sheds light on the most prevalent hazards in the Queensland mining industry for FY24. Understanding these hazards is crucial for implementing effective safety measures and ensuring the well-being of all workers. 🔍 Key Insights: 🔹 Fire tops the list as the most common hazard, emphasising the need for comprehensive risk assessments of potential ignition sources, fire prevention controls, and emergency preparedness for fire events. 🔹 Vehicle Collisions continue to persist in the industry. Vehicle interaction is a major hazard at all mining operations and requires the effective implementation of robust controls that go beyond purely administrative and behavioural-based measures. 🔹 Fall of Personnel/Equipment and Material Incidents are still prevalent in the industry. Ensuring that hazards associated with suspended loads and working at heights are appropriately controlled and managed is a fundamental expectation of an effective SHMS. 🔹 Exposure to Inorganic Materials/Hazardous Substances is another common risk area where incidents are preventable through effective risk management and controls. 🔹 Rollover Incidents involving mobile plant and vehicles remain a high-potential risk that, alarmingly, continues to persist in our industry. The MMQ Quarterly 🔹 Report details an HPI involving a crane rolling into a tailings pond, providing critical insights into this very concerning hazard. If you or your organisation need help in identifying, assessing, or controlling any of these hazards, or ensuring that your Safety and Health Management System is as effective and robust as it can be, then contact one of our team for a discussion and advice. Our exploration efforts are in full swing across the EinGold and Ravenswood projects, where we're drilling over 5 kilometres of Reverse Circulation and Diamond holes to refine our geological models in these promising gold and copper regions. It was great to have our Country Manager Australia, Andrew Hackwood, on-site to inspect the drilling operations, which are being expertly overseen by Lennard Kolff van Oosterwijk , our newly appointed General Manager of Minerals. Vidya Harikumar, Director of People and Culture, and James Huxtable, Manager of Contracts and Procurement, also joined the site tour, witnessing firsthand the impact of their hard work in Corporate Services. A big shoutout to DRC Drilling for providing the Reverse Circulation and Diamond Rigs, SSE Co. for their operational expertise as Operator, and Xplore Resources Pty Ltd for their ongoing support and off course our amazing Minerals team Michael Tschaban and Jesse Savage for all the hours, planning and execution that had gone behind this project! #Teamwork #Exploration #Mining #Geology #TriStar #Jobs #StrategicMaterialsList

RSHQ has released Safety Bulletin #222, highlighting an emerging pattern of reported High Potential Incidents (HPIs) related to the uncontrolled fall of plants from stands at mining operations. While no Coal Mine Workers were injured, these incidents had the potential to result in serious injury or fatality. An animation video has also been released to help with understanding what went wrong and the recommendations to prevent recurrence. 🔍 Incident Overview: A series of HPIs involving the uncontrolled fall of plant from jack stands and similar equipment have been reported across multiple mining sites. The key issues identified include: 🔹 Planning: Inadequate planning and failure to refer to OEM manuals and procedures. 🔹 Risk Assessment: Risk assessments and risk-based procedures were inadequate; key hazards such as line of fire and suspended loads were not identified. 🔹 Jacks, Stands, and Dollies: Jacking procedures were inadequate, and OEM procedures were not considered. Appropriate stands were not in place. Supervision: Supervision was ineffective, with a lack of oversight for assessing high-risk tasks, relying instead on assumed prior experience and knowledge. 🔹 Training: Coal Mine Workers had limited understanding of equipment such as jacks, stands, and dollies/saddles. Key Takeaways: 🔹 Work Planning and Risk Management: Ensure risk-assessed work planning processes, including reference to OEM manuals and procedures. Fit-for-Purpose Jacks, Stands, Saddles, and Dollies: Provide appropriate tools for all maintenance tasks. 🔹 Training and Ongoing Assessments: Implement processes to verify the competency of Coal Mine Workers, including ongoing verification. 🔹 Communication: Share the animation video with Coal Mine Workers involved in jacking and standing, and ensure procedures are communicated to all relevant personnel. The latest MMQ Quarterly Report highlights a significant incident involving work on a slurry pump that was not electrically isolated. This incident underscores the crucial importance of proper isolation procedures to ensure worker safety. 🔌 Incident Overview: 🔹 Workers commenced repairs on a slurry pump without verifying its electrical isolation. 🔹 The Safe Work Instruction (SWI) lacked a process to identify and confirm the isolation of the pump. 🔹 Identical pumps in the same location with unclear labelling led to confusion. 🔍 Key Takeaways: 🔹 Clear Labelling and Marking: Ensure all equipment is clearly marked to prevent confusion. 🔹 Verification of Isolation: Implement a robust verification process to confirm that equipment is correctly isolated before commencing work. 🔹 Proper Training: Provide comprehensive training to all workers on isolation procedures and the importance of 'test for dead' protocols. This incident highlights the need for stringent safety measures and adherence to isolation procedures to prevent similar occurrences. Continuous improvement in safety protocols and worker training is essential to maintain a safe working environment. Safety in mining is often more defined by its absence than its presence—a phenomenon known as the Safety Paradox. The success of safety measures is not always visible, making it crucial to focus on lead indicators rather than lagging ones. This approach aligns with the principles of High Reliability Organisations (HROs), which prioritise proactive measures and continuous improvement. At SSE Co, we are committed to advancing safety culture in the mining industry. Our latest blog delves into the intricacies of the Safety Paradox, exploring why traditional safety metrics can sometimes fall short and how focusing on lead indicators can provide a more accurate picture of safety performance. Are you effectively leveraging lead safety indicators in your operations? Understanding and implementing these measures can significantly enhance your organisation's safety culture. At SSE Co, we enjoy discussing these issues and assisting in finding better ways to improve safety.
