Project ECHO ISC – Building Confidence in Dementia Care Dementia is a growing concern in Canada’s aging population, and many care providers express uncertainty about how to best support their patients. Project ECHO Integrated Seniors Care (ISC) is designed to bridge this confidence gap by providing hands-on learning and expert-led sessions that build the skills necessary to care for seniors with complex chronic conditions. Understanding the Challenges: Caring for seniors living with dementia is complex. A survey of home care providers revealed that only 9% felt adequately prepared to handle behavioural changes in patients with dementia. In comparison, 57% of primary care physicians cited time constraints as a significant barrier to managing dementia patients. Project ECHO ISC will help address these challenges by creating a space for collaborative learning, knowledge sharing, and practical skill development. Learning Clinics Focused on Dementia: In 2024, Project ECHO ISC is focusing on Mild Cognitive Impairment (MCI) and Dementia, offering primary care and home care providers the opportunity to engage in a series of TeleECHO Learning Clinics. The first of these sessions will take place on October 1 and cover the early identification of cognitive decline, helping providers recognize when it’s time to initiate further screening and diagnosis. These sessions are designed to teach and foster a network of providers who can support each other in delivering patient-centred, coordinated care for seniors with dementia. The Path Forward: Project ECHO ISC is a vital initiative for transforming primary and home-based care in Canada. By enhancing provider knowledge and building confidence, we can ensure that seniors living with dementia are cared for with the respect, dignity, and compassion they deserve. Project ECHO Integrated Seniors Care – Addressing the Knowledge Gaps in Dementia Care As Canada’s population continues to age, the need for specialized care for seniors with complex chronic conditions, such as dementia, is becoming more urgent. Project ECHO Integrated Seniors Care (ISC), the initiative led by the Canadian Home Care Association (CHCA) in partnership with the Canadian Medical Association, aims to address the gaps in training, knowledge, and coordination that many primary care and home care providers face when caring for seniors with complex chronic conditions. Bridging Knowledge Gaps Through Collaborative Learning: The ISC program offers a collaborative learning model through expert-led presentations and case-based learning. Care providers across primary and home care settings join interactive sessions to gain hands-on knowledge that can be immediately applied to their practices. By breaking down silos and encouraging multidisciplinary engagement, Project ECHO ISC aims to improve care coordination and enhance the confidence of healthcare professionals. 2024 Theme: Mild Cognitive Impairment and Dementia: In 2024, Project ECHO ISC will focus on the growing issue of mild cognitive impairment (MCI) and dementia. With over 58% of people living with dementia receiving home care at some point during their care journey, there is a significant opportunity to enhance care practices across the country. Participants in the TeleECHO Learning Sessions will dive deep into risk factors, early identification, routine cognitive screening, and personalized care planning to improve the lives of seniors with MCI and dementia. Why It Matters Now: The need for integrated, person-centred care has never been more critical. With many care providers needing more preparation to handle complex conditions, Project ECHO ISC offers a sustainable model for increasing competency in a challenging yet vital care area.
