The World Health Organization (WHO) and the Global Alliance for Surgical, Obstetric, Trauma, and Anaesthesia Care (G4 Alliance) signed a Memorandum of Understanding (MoU). This MoU is a testament to their shared commitment to enhancing emergency, critical, surgical, obstetric, trauma, and anaesthesia care all over the world, especially in low—and middle-income countries. This partnership, a significant milestone for the G4 Alliance and towards achieving Universal Health Coverage (UHC), is focused on several key areas of significant importance: - UHC: Focusing on the role of operative care as an integral part of primary healthcare systems, especially in low and middle-income countries. - Dissemination of WHO Tools: Upscaling the dissemination of WHO's clinical registry and quality improvement tools to enhance healthcare providers' capabilities in delivering operative care efficiently. - Advocacy and Fundraising: Supporting advocacy efforts, creating a community of practice, and undertaking fundraising activities to increase the adoption of quality improvement platforms, particularly in resource-limited settings. - Data Collection and Monitoring: Improving outcomes and quality of operative care services through continuous data collection, analysis, and monitoring for service development and quality improvement. Claudio Tanca, Executive Director of G4 Alliance, said, "We are thrilled to join forces with WHO. Our shared commitment to advancing surgical, obstetric, trauma, and anaesthesia (SOTA) care is critical in achieving universal health coverage. Together, we aim to make a lasting impact on the lives of millions of patients in low and middle-income countries." We look forward to working closely with WHO to realize the objectives outlined in the MoU. Thank you to all our members who encouraged and supported the G4 Alliance during the entire process. Your continued dedication is crucial as we strive to improve the quality of and access to SOTA care globally. We are the G4 Alliance! #SOTACare4All #GlobalHealth #UniversalHealthCoverage #Surgery #Obstetrics #TraumaCare #Anaesthesia #HealthcareCollaboration #G4Alliance #WHO

A new clinical trial aimed at improving the treatment of elbow (supracondylar humerus) fractures in children has been launched. The trial, spearheaded by Dr Mengistu Gebreyohanes Mengesha, will compare two treatment methods: a conservative approach using traction and a surgical method. The goal is to identify the most cost-effective and sustainable way to treat these common and often complicated fractures in children. This initiative, supported by the AO Alliance, brings together 12 pediatric trauma and orthopedic surgeons as clinical investigators from nine major hospitals across Ethiopia. Beginning in August 2024, the two-year trial will recruit 222 patients. This study has the potential to set a new standard for pediatric orthopedic care in Ethiopia and other low-resource settings.

The AO Alliance continues to advocate for improved care for trauma and injury victims in low- and middle-income countries (LMICs) in Africa and Asia, contributing to international forums that champion change. The AO Alliance joined the WHO Global Alliance for Care of the Injured (GACI) at its annual meeting in Geneva, Switzerland. The event centered on World Health Assembly Resolution 76.2 on “emergency, critical and operative care” (ECO), recognizing that robust ECO services are essential to the ability of national health systems to respond effectively to emergencies and injuries. They are also integral to achieving Universal Health Coverage under the UN Sustainable Development Goals. The AO Alliance and other members were invited to take part in the development of the strategic and implementation plans of the ECO resolution. This resolution marks a significant step towards global recognition of the important, yet often overlooked, issue of injuries and access to care, underscoring the need for its inclusion in national public health agendas worldwide. Dr Claude Martin Jr, AO Alliance Managing Director, gave the keynote lecture at the Annual General Meeting of the Ethiopian Society of Orthopaedics and Traumatology (ESOT) on June 7-8, 2024 in Addis Ababa, Ethiopia. The meeting covered a variety of topics, from the prevalence of traditional bonesetting practices to robotic total knee replacement techniques. Dr Martin’s lecture, titled The Impact of Musculoskeletal Injuries in Low- and Middle-Income Countries (LMICs), shed more light on the widespread lack of access to timely and appropriate injury care and proposed a systems based approach to dealing with the rising burden of injuries. At the 14th Best Medical Practices Forum on June 16-18, 2024, in Arusha, Tanzania, the resolution to adopt the AO Alliance/COSECSA clinical guidelines for three of the most common fractures was adopted by both the East, Central and Southern Africa Health Community (ECSA-HC) and the health ministers of nine Sub-Saharan countries in attendance. The guidelines for closed femoral shaft fractures, closed ankle fractures, and open tibia shaft fractures, adapted to LMICs, will advance and streamline care of the injured, leading to improved patient outcomes and cost-effectiveness for health systems in LMICs. The AO Alliance would like to acknowledge the College of Surgeons of East, Central, and Southern Africa (COSECSA) and its CEO, Stella Itungu, for their support and commitment to getting the guidelines adopted. Access the guidelines here -> https://ow.ly/KALK50SFXJj The AO Alliance's active participation in such key events exemplifies its dedication to improving care of the injured in LMICs by advocating for systemic changes and fostering collaborations to ensure better health outcomes for all. #COSECSA

"In response to the Political Declaration of the High-Level Meeting on Universal Health Coverage (UHC), world leaders must urgently prioritize concrete action to close the healthcare gap for the 4.5 billion people who lack access to essential health services, particularly for fractures. Access to timely and appropriate fracture care is integral to overall health and well-being, and addressing this issue is critical to achieving UHC goals. First, leaders should invest in building robust health infrastructures that ensure equitable distribution of health facilities, including trauma centres, and rehabilitation services. This requires strategic planning and resource allocation to reach underserved populations, especially in remote areas where access to health facilities may be limited. Second, a comprehensive approach to financial protection is critical. Leaders should explore innovative financing models, such as health insurance schemes or community-based financing mechanisms, to reduce the financial burden on individuals seeking fracture care. This includes working with the private sector and international organizations to create sustainable financing solutions that protect individuals from catastrophic healthcare costs. In addition, prioritizing education and awareness programs can empower communities to prevent fractures and seek timely medical care. This includes campaigns on injury prevention, proper safety measures, and the importance of early intervention for fractures. Finally, promoting international collaboration is essential. Leaders should engage in partnerships with other nations, non-governmental organisations, and global health initiatives to share best practices, resources, and expertise in addressing the unique challenges associated with fracture care. By taking these meaningful steps, leaders can make considerable progress towards ensuring universal access to quality fracture care, contributing significantly to the broader goals of universal health coverage, and leaving no one behind." - Dr Claude Martin Jr, Managing Director, AO Alliance #HealthForAll #UHCDay #UniversalHealthCoverage The G4 Alliance UHC2030 - Taking action for universal health coverage OECD - OCDE United Nations
