𝐏𝐫𝐨𝐬𝐭𝐚𝐭𝐞 𝐂𝐚𝐧𝐜𝐞𝐫 𝐚𝐧𝐝 𝐄𝐫𝐞𝐜𝐭𝐢𝐥𝐞 𝐃𝐲𝐬𝐟𝐮𝐧𝐜𝐭𝐢𝐨𝐧 Prostate cancer is a prevalent disease, affecting one in five American men. While treatments like surgery, radiation, and hormone therapy can be life-saving, they often come with significant side effects, including erectile dysfunction (ED). When Can ED Occur After Prostate Cancer Treatment? Some degree of ED typically occurs immediately following surgery, even with nerve-sparing techniques. Surgery can lead to nerve damage, arterial damage, and tissue fibrosis, which causes venous leakage. Recovery can take up to two years if nerves are spared, though recovery is less likely with non-nerve-sparing procedures. Radiation therapy presents a different timeline, with ED usually developing gradually, starting around six months post-treatment. Radiation therapy might spare the erection nerves but can still cause arterial damage and tissue fibrosis. Modern techniques such as brachytherapy, IMRT, and 3-D conformal radiotherapy can reduce the risk but do not eliminate it. When it comes to hormone therapy, Androgen deprivation therapy (ADT) can lead to smooth muscle dysfunction, tissue fibrosis, and hypoactive sexual desire. ED and reduced sexual desire can begin within two to four weeks due to testosterone suppression. Androgen deprivation therapies like Lupron and Zoladex are effective in lowering testosterone levels, but they can also cause significant sexual dysfunction. A landmark study by Schover et al. highlighted the challenges faced by prostate cancer survivors. The survey of 1,236 men revealed that 36% had ED before treatment, which increased to 85% post-treatment. Only 13% reported having reliable, firm erections spontaneously, and just 8% achieved erections with medical aid. An article from Boston University School of Medicine highlights that this condition is not just an inconvenience, it significantly impacts the quality of life and overall health. ED is seen in 28% of diabetic men versus 9.6% in non-diabetic men. The incidence of ED in diabetic men rises from 15% at age 30 to 55% at age 60. Poor diabetes control and associated complications like vascular and nerve diseases amplify the risk. Diabetes affects penile functions through several mechanisms including psychological factors, central and peripheral nervous system damage, hormonal imbalances, and blood vessel and smooth muscle dysfunction. Arterial blockages are 40 times more prevalent in diabetic men, causing decreased rigidity and frequency of nighttime erections. Electron microscope studies reveal less smooth muscle, more scar tissue, and fewer endothelial cells in diabetic men.
