What happens if you eliminate payments for pain and suffering from compensation claims in whiplash? This revealing study from Canada from 2000 looked at the effect of bringing in a no-fault compensation system, where there was previously a tort system that provided for payments for pain and suffering. Comparing the number of claims made and the recovery time before and after no-fault compensation was introduced, the authors found that; 1. The incidence of claims dropped by 43% for men and 15% for women. 2. The median time from bringing the claim to closure reduced by 56% for men and 54% for women. The authors concluded that “The elimination of compensation for pain and suffering is associated with a decreased incidence and improved prognosis for whiplash injury.”

Review of the Therapeutic benefits of cervical facet joint interventions. This review paper from Pain Physician 2012 analyses studies on radiofrequency neurotomy, medial branch blocks and facet joint injections in patients with chronic neck pain. Convincing evidence is well designed studies is classified as Good. Fair means there is some evidence that the treatment is effective, but the studies relating to such are not fully robust. Limited means there is insufficient evidence of the effectiveness of the treatment and a lack of robust research on it. The final determination for radiofrequency neurotomy and medial branch blocks was Fair and for facet joint injections it was Limited. One has to question, therefore, the ongoing widespread and repeated use of these treatments for this condition. GleeMed www.gleemed.ie
