Some fractures can be missed on conventional X-rays, especially in areas of complex anatomic structure such as the wrist or ankle. The cast obscures some of the anatomy, and in many cases the patient is asked to remove the cast before the imaging creating discomfort for the patient. These fractures can be seen more clearly when captured through digital tomosynthesis, and the effect of the cast is minimal - hence it is unnecessary to remove the cast.
