💡 Publication Alert! 🎇 Excited to announce that our article ``Building Health Policy and Systems Research (HPSR) capacity in India: Reflections from the India HPSR fellowship program (2020-2023)" by the Health Policy and Systems Research (HPSR) Secretariat and Health Systems Transformation Platform (HSTP) team has now been published in #BMC. 🔍#HPSR focuses on understanding and improving how societies organize to achieve collective health goals by examining interactions in policy and implementation processes. Building #HPSR capacity in lower- and middle-income countries (LMICs) is essential for generating evidence that strengthens health systems, enhances policies, and supports better health outcomes. 📍 The India HPSR Fellowship, anchored at Health Systems Transformation Platform, is a capacity-building initiative that works to address this need. Developed in collaboration with institutes both in India and internationally, the fellowship has already trained three cohorts of fellows between 2020 and 2023. 💡 This article provides an overview of the fellowship and reflects on the key learnings from its early years. Notably, globally available #HPSR curricula need to be adapted to country-specific contexts, and unlearning prior assumptions is crucial for fellows. The program must also address challenges like publication barriers and work obligations. Moreover, it highlights the need for stronger research–policy–practice interfaces to enhance the policy translation of fellows' work. However, sustaining such fellowships in LMICs remains challenging due to high costs and limited immediate outputs. Home Healthcare (HHC) is gaining popularity in India due to the increasing elderly population, rising prevalence of chronic conditions, changing family structures, and advancements in healthcare technology. The COVID-19 pandemic further highlighted the expanding size and scope of HHC in the country. However, Health Systems Transformation Platform's report, "Regulation of Health Care Delivery in India – A Landscape Study," which reviewed central laws regulating healthcare delivery in the country, highlighted the lack of clarity on what constitutes HHC, how it is organized and delivered, and the corresponding regulatory structures. Effective research management drives research, fosters innovation, and cultivates a strong research culture. Providing institutional support for partnership building, grant management (pre- and post-award), and compliance oversight enables researchers to access diverse funding sources. Though still emerging in India, research management is increasingly recognized as a vital scientific administrative discipline, offering growing career opportunities for research managers and administrators (RMAs). The #IndiaResearchManagementInitiative (IRMI) by DBT/WT India Alliance serves as a platform for the growing RMA community to connect and collaborate, further strengthening research management efforts nationwide. Health Systems Transformation Platform recently participated in the 12th DBT/WT India Alliance Conclave, held from August 28-30, 2024, in #Hyderabad, Telangana. The Conclave offered a significant platform for this crucial discourse by bringing together researchers, fellows, reviewers, stakeholders, and donors. It facilitated grant reviews, knowledge sharing, and cross-disciplinary collaborations. The Conclave showcased groundbreaking scientific research and strategic discussions on research management. Kicked off with high-energy lightning talks by Early Career Fellows, the event featured 105 poster presentations from 70 institutions. Topics ranged from molecular mechanisms and injury repair to innovations in heart failure treatment, cancer immunotherapy, and the genetics of psychiatric disorders, highlighting the diverse research supported by the India Alliance. The Conclave was attended by over 100 awardees, all participating in this review process. Imagine a healthcare system where no female health worker in a vibrant pink saree walks through the lanes of Mysuru’s urban slums, knocking on the door to remind you about your child’s immunization or checking if the pregnant woman in your household has taken her iron supplements. There’s no one to gently encourage a young couple to consider family planning or to help a pregnant woman understand the importance of four ANC visits. Where is that friendly face who ensures that mothers and children receive vital healthcare? The lanes once filled with the purposeful strides of these health warriors, are now eerily silent. She is the Accredited Social Health Activist (#ASHA)- a health advocate, a mobilizer, and a counselor- donning many roles! Without ASHAs, the pregnant women, who would typically crowd the UPHCs for their routine #ANC visits, would be fewer in numbers. ASHAs, who tirelessly visit homes to ensure these women take their #IFA tablets- will miss their essential doses. What about the #UHND & Health Camps? These camps where ASHAs and AWW, ANMs collaborate to share critical information on RMNCH+A shall be a distant memory. The vibrant gatherings of community members learning about health and wellness are likely to be replaced by quiet corners where misinformation spreads unchecked. Without ASHAs, the risk of complications during childbirth shall escalate, and worried families replace the once-constant hum of health discussions. #Couples relying on ASHAs for contraceptive counseling can be seen struggling with unplanned pregnancies, adding strain to already limited resources. In a close-knit community, the #RoutineImmunization program is likely to falter, leaving children vulnerable to vaccine-preventable diseases like measles, polio, and diphtheria. As families forget to avail of timely vaccines, the threat of these diseases reemerges, erasing years of progress and spreading suffering. The absence of ASHA workers would create a chasm between the government’s health programs and the communities they serve, causing critical health service delivery to plummet and leaving many without essential care. India's healthcare system, particularly for the most #vulnerable, would become distant, and #inaccessible. Health Systems Transformation Platform salutes these foot soldiers working tirelessly in #Mysuru UPHCs to enhance the RMNCH+A outcomes for their unwavering commitment. Their stories of triumph, navigating the complexities of healthcare delivery in challenging ecosystems serve to be celebrated and documented.
